Educational Intervention Reduces Utilization of Low-Value Testing in Cerebrovascular Inpatients: An Example of Rapidly Implementable Quality Improvement (P2.309)
Conclusions: In this prospective quality improvement study, an educational intervention resulted in significant reduction in utilization of low-value tests in patients admitted to an inpatient cerebrovascular service. While the per-patient costs of these diagnostic tests were small, a similar approach at the system level could result in considerable avoidance of low value testing and associated costs, particularly if applied to higher-cost/lower-value investigations and procedures.Disclosure: Dr. Scharf has nothing to disclose. Role of the Stroke Unit in Reducing the Medical and Neurological Complications Post Stroke in Patient Admitted at Royal Hospital from 2009-2015 (P2.318)
CONCLUSIONS: Medical complications that follow acute stroke not only influence mortality but may influence functional outcome. Stroke patients who receive organized inpatient care in a stroke unit are less likely to be developing medical complications post strokeDisclosure: Dr. Al-Hashmi has nothing to disclose. Intracerebral Hemmorhage Transfers a Comprehensive Stroke Center Experience (P2.319)
Conclusions: Contrary to our expectation transferred patients show a trend towards better outcomes compared to directly admitted patients. They had lower ICH scores, which probably explains this better outcome. Our results raised the need to analyze cost, benefits and resource utilization off transferring patients with milder ICH.Disclosure: Dr. Mir has nothing to disclose. Risks and Benefits of Bypassing the Emergency Department for Ischemic Stroke Cases Transferred for Endovascular Therapy (P2.320)
Conclusions: In our sample of acute ischemic patients transferred for acute endovascular therapy, nearly 60[percnt] of patients bypassed the ED, reducing time to recanalization. This time savings was associated with a clinically higher proportion of adverse events. Standardized protocols for patients transferred to acute ET are needed to reduce time to recanalization without increasing adverse event rates.Disclosure: Dr. Kaur has nothing to disclose. Non-Invasive Vascular Imaging Before Endovascular Therapy: Time Wasted or Brain Saved? (P2.331)
Conclusions: Our observations suggest that HDV sign may serve as a proxy for NIVI in identifying large vessel occlusion, reducing delays associated with NIVI. Bypassing the recommended NIVI may, however, be associated with higher proportion of HT following ET. Larger prospective studies are needed to further investigate this clinical conundrum and determine if these findings are confounded by the ASPECTS score on the initial head CT.Disclosure: Dr. Jones has nothing to disclose. Dural Arteriovenous Fistula, Medullary Edema, and Ondines Curse: A Case Report (P3.195)
Conclusions: New onset respiratory failure due to dysfunction in the CNS, called acquired central alveolar hypoventilation, has been seen in several common conditions that may affect the medulla, including demyelinating lesions and stroke. We report the first case of acquired central alveolar hypoventilation due to dAVF, where apnea during sleep (was the patient’s only residual neurologic deficit after treatment, the so-called Ondine’s curse. Clinicians evaluating hypercapnic respiratory failure should consider brainstem pathology that may impair central respiratory function and consider a dedicated angiography...

Using Accelerometers in the Neurological ICU to Monitor Unilaterally Motor Impaired Patients (P3.204)
Conclusions: Continuous monitoring of limb activity with accelerometers is feasible and can be used to identify motor asymmetry after neurological injury. From 'Sweet Clover Disease' to NOACs: A Historical Perspective on Oral Anticoagulation for Prevention of Cardioembolic Stroke (P2.397)
CONCLUSIONS: Warfarin, born from ‘sweet clover disease’ and humble beginnings as rodenticide, has been the cornerstone of cardioembolic stroke prevention. Its shortcomings inspired the development of a new generation of oral anticoagulants.Disclosure: Dr. Chiota-McCollum has nothing to disclose.

Non-traumatic Cerebral Fat Embolism in Sickle Cell Disease (P3.234)
Conclusions:Our case series illustrates the following clinical pearls: a) SCD-related CFES is vastly under-diagnosed, as it closely mimics sepsis, vasculitis or hypoxic ischemic encephalopathy; b) The diagnosis is confirmed with SWI that demonstrates pathognomonic microbleeds in the splenium and subcortical location; c) SCD-related CFES carries considerable morbidity and mortality.Disclosure: Dr. Ramachandiran has nothing to disclose.
