Design of Randomized, double‐blind, Evaluation in secondary Stroke Prevention comparing the EfficaCy and safety of the oral Thrombin inhibitor dabigatran etexilate vs. acetylsalicylic acid in patients with Embolic Stroke of Undetermined Source (RE‐SPECT ESUS)
DiscussionAcetylsalicylic acid is the most common antithrombotic given to patients with embolic strokes of undetermined source to reduce recurrence risk. This trial will determine whether anticoagulation with dabigatran is more effective than acetylsalicylic acid, and acceptably safe.

Is acute reperfusion therapy safe in acute ischemic stroke patients who harbor unruptured intracranial aneurysm?
ConclusionOur findings suggest that neither intravenous thrombolysis nor combination of intravenous and intra‐arterial thrombolysis increases the risk of aneurysmal hemorrhage in acute ischemic stroke patients who harbor unruptured intracranial aneurysms less than 10 mm in diameter. Their listing in exclusion criteria for intravenous thrombolysis should be reconsidered to assure appropriate use of acute reperfusion therapy in this group of patients.

A collaborative sequential meta‐analysis of individual patient data from randomized trials of endovascular therapy and tPA vs. tPA alone for acute ischemic stroke: ThRombEctomy And tPA (TREAT) analysis: statistical analysis plan for a sequential meta‐analysis performed within the VISTA‐Endovascular collaboration
DiscussionThis collaborative meta‐analysis of individual participant data from randomized trials of endovascular therapy vs. control in conjunction with intravenous thrombolysis will demonstrate the efficacy and generalizability of endovascular therapy with intravenous thrombolysis as a concomitant medication.

Effect of helicopter transport on neurological outcomes in a mouse model of embolic stroke with reperfusion: AIR‐MICE pilot study
ConclusionsThis study shows that helicopter emergency medical services do not have an inherent adverse effect on outcome in a mouse model of ischemic stroke with reperfusion. These results endorse the safety of the practice of using helicopter emergency medical services in stroke patients. The observed potential synergistic effect of helicopter‐induced factors, such as vibration and changes in altitude, with reperfusion merits further exploration in animal experimental models and in stroke patients.

The correlation between admission blood glucose and intravenous rt‐PA‐induced arterial recanalization in acute ischemic stroke: a multi‐centre TCD study
ConclusionHyperglycemia is associated with low rate of complete recanalization and poor clinical outcome in intravenous recombinant tissue‐plasminogen activator‐treated patients. Further studies are needed to evaluate whether lowering hyperglycemia is beneficial in the management of acute stroke patients.

Posterior vs. anterior circulation infarction: demography, outcomes, and frequency of hemorrhage after thrombolysis
ConclusionsOur study suggests that posterior circulation stroke is associated with a lower risk of intracranial hemorrhage than anterior circulation stroke is.

A systematic review and meta‐analysis of randomized controlled trials of endovascular thrombectomy compared with best medical treatment for acute ischemic stroke
ConclusionsThere is clear evidence for improvement in functional independence with endovascular thrombectomy compared with standard medical care, suggesting that endovascular thrombectomy should be considered the standard effective treatment alongside thombolysis in eligible patients.

Acute Stroke or Transient Ischemic Attack Treated with Aspirin or Ticagrelor and Patient Outcomes (SOCRATES) trial: rationale and design
DiscussionThe SOCRATES trial fulfills an important clinical need by evaluating a potent antiplatelet agent as a superior alternative to current standard of care in patients presenting acutely with ischemic stroke or transient ischemic attack.

High rate of microbleed formation following primary intracerebral hemorrhage
ConclusionsWe found that one‐third of intracerebral hemorrhage subjects in this cohort surviving one‐year developed new microbleeds, which suggests a dynamic and rapidly progressive vasculopathy. Future studies are needed to examine the impact of new microbleed formation on patient outcomes.

Statin use in spontaneous intracerebral hemorrhage: a systematic review and meta‐analysis
ConclusionsAlthough careful interpretation is necessary due to several study limitations, we have demonstrated that statin use in patients with intracerebral hemorrhage is likely associated with improved mortality and functional outcomes.
