Depression and Incident Cardiovascular Disease —Reply
In Reply To characterize the association between depressive symptoms and CVD incidence, we analyzed individual participant data from 22 prospective studies that included baseline information about self-reported depressive symptoms in participants without a history of CVD. Our study demonstrated modest log-linear associations between baseline depressive symptoms and future risk of coronary heart disease, stroke, and CVD. Associations were present even with symptoms below the threshold indicative of depressive disorder and were unaffected by adjustment for several established CVD risk factors. These results are consistent wi...

Functional Outcomes Among Patients With Acute Ischemic Stroke After Mechanical Thrombectomy With or Without Intravenous Thrombolysis —Reply
In Reply Dr Mai and colleagues describe some concerns with the SKIP study, including the balance of baseline characteristics between the 2 groups, the sample size, and the noninferiority margin for the secondary outcome.

Effect of Continuous Cardiac Monitoring vs Usual Care on Detection of Atrial Fibrillation
This randomized clinical trial examines whether long-term cardiac monitoring with an insertable cardiac monitor was more effective than usual care for detection of atrial fibrillation in patients with stroke attributed to cervical or intracranial large artery atherosclerosis or small-vessel occlusion (large- or small-vessel disease).

The USPSTF Recommendation Statement About Screening Asymptomatic Adults for Carotid Stenosis
To the Editor The USPSTF recommends against screening asymptomatic adults for carotid stenosis. However, we believe that detection of asymptomatic carotid stenosis might encourage treatment adherence to pharmacological —and nonpharmacological—primary prevention strategies. Demonstration of an objective lesion in the carotid artery may increase patient awareness of the presence of a risk factor for stroke. One article demonstrated that a higher rate of carotid stenosis detection was associated with better adher ence to statin and aspirin treatment in asymptomatic patients with vascular risk factors. A letter commenting ...

TAVR for Bicuspid Aortic Valve Stenosis
This registry-based cohort study compared rates of mortality and stroke at 30 days and 1 year among patients at low surgical risk with bicuspid vs tricuspid aortic stenosis undergoing transcatheter aortic valve replacement (TAVR).

Thrombectomy With Combined Contact Aspiration and Stent Retriever vs Stent Retriever Alone
This randomized clinical trial compares the effect of thrombectomy with contact aspiration and stent retriever combined vs stent retriever alone on revascularization in patients with acute ischemic stroke and large vessel occlusion.

Implantable vs Prolonged External Electrocardiographic Monitoring for Atrial Fibrillation Detection in Ischemic Stroke
To the Editor In a recent study, implantable electrocardiographic monitoring for 12 months resulted in detection of a significantly greater proportion of patients with atrial fibrillation compared with prolonged external monitoring for 30 days. The diagnostic yield of these 2 methods may be due to adherence as well as to the function of the monitoring device itself.

USPSTF Review: Screening for Atrial Fibrillation
This systematic review to support the 2022 US Preventive Services Task Force Recommendation Statement on screening for atrial fibrillation (AF) summarizes published evidence on the benefits and harms of screening and anticoagulant therapy among asymptomatic persons without known AF or prior stroke.

Strokes Occur Later and Are Less Severe Than 2 Decades Ago
A registry study of patients in Japan suggests that all stroke types have become less severe over the past 2 decades and are occurring later in life. Short-term functional outcomes also improved for patients with ischemic strokes. The findings appeared in JAMA Neurology.

Intra-arterial Thrombolysis to Target Occlusions in Distal Arteries and the Microcirculation
Acute ischemic stroke remains a leading cause of disability in the United States and worldwide despite substantially improved treatments for the most severe strokes over the last 5 years. Treatment of severe ischemic strokes, specifically those due to occlusion of large intracranial arteries, has been transformed by endovascular removal of the thrombus using stent retrievers or aspiration catheters. Treatment with mechanical thrombectomy is associated with a 20% to 27% absolute increase in the rate of functional independence compared with patients not treated with thrombectomy. Despite highly efficacious treatment, approxi...

Self-monitoring of Blood Pressure During Pregnancy
The COVID-19 pandemic has led to the frequent use of telemedicine and the common recommendation for self-monitoring of blood pressure (SMBP) during pregnancy for individuals at risk for or with hypertensive disorders of pregnancy, as an adjunct to or substitute for some aspects of in-person prenatal care visits. BP monitoring is a critical aspect of prenatal care to screen for preeclampsia. The maternal and neonatal morbidities associated with hypertension in pregnancy are well-established, including risks of preterm delivery, low birth weight, preeclampsia, and rare risks of maternal stroke, seizure, and death. It is unce...
