The impact of hypoalbuminemia on postoperative outcomes after outpatient surgery: a national analysis of the NSQIP database
ConclusionsHypoalbuminemia is associated with major complications and death in outpatient surgery. Since hypoalbuminemia is a potential modifiable intervention, future clinical trials to evaluate the impact of optimizing preoperative albumin levels before outpatient surgery are warranted.

Safety of SARS-CoV-2 vaccination in patients with Behcet's syndrome and familial Mediterranean fever: a cross-sectional comparative study on the effects of M-RNA based and inactivated vaccine
Rheumatol Int. 2022 Apr 4. doi: 10.1007/s00296-022-05119-y. Online ahead of print.ABSTRACTMost of the published data relate to classical forms of rheumatic diseases (RD) and information on rare inflammatory disorders such as Behçet's syndrome (BS) and familial Mediterranean fever (FMF) is limited. We studied the frequency of side effects and disease flares after COVID-19 vaccination with either Pfizer/BioNTech or Sinovac/CoronaVac in 256 patients with BS, 247 with FMF, and 601 with RD. Telephone interviews were conducted using a questionnaire survey in a cross-sectional design in patients with BS, FMF, and RD followed by ...

The venous system during pregnancy. Part 1: physiologic considerations on the venous system
The confluence of anatomic and physiologic gestational changes, pre-existing and pregnancy-related comorbidities, surgery and anesthesia has significant implications for maternal hemodynamics. Genetic and hemodynamic factors alter endothelial migration and branching during embryogenesis to create separate venous and arterial systems, 1 which subsequently possess distinct responses to disease processes (e.g. venous insufficiency, thromboembolism, stroke). 2

Eagle Syndrome: A Rare Cause of Stroke in a Young Patient
We report the case of a 26-year-old man who presented to the emergency department because he noticed that his right hand had become clumsy. He first noticed this symptom five days before his presentation, but he noticed that his symptom had improved significantly since it began. He reported that he had episodes of neck pain and pain around the ear. He visited the family physician clinic several times for this complaint and was diagnosed as having a temporomandibular joint disorder. Neurological examination revealed decreased muscle strength in the right upper limb with a power of 4/5 along with a sensory deficit. The coord...

Postpartum cerebral venous sinus thrombosis following obstetric neuraxial blockade: a literature review with analysis of 58 case reports
Cerebral venous sinus thrombosis (CVST) is an uncommon but serious complication of pregnancy.1 Pooled analysis from a recent systematic review revealed that CVST is one of the major causes of stroke in pregnancy.2 Confidential maternal death enquiry reports in the United Kingdom (UK) have identified 12 deaths from CVST across the last 15 years.3 –5 The postpartum period, especially the first six weeks following delivery, is now considered an independent risk factor for CVST, with the risk 3.5 times higher when compared with non-pregnant women.

Neurosurgical intervention in children with ventricular assist devices: A single center case series review
CONCLUSIONS: Perioperative concerns for the anesthesiologist include VAD hemodynamic management, bleeding, VAD thrombosis, and prevention of secondary brain injury. A systematic, multidisciplinary approach to management is paramount to attain favorable outcomes.PMID:34478592 | DOI:10.1111/pan.14287

A wolf in sheep's skin? Postoperative atrial fibrillation after cardiac surgery and the risk of stroke and mortality
Postoperative Atrial Fibrillation (poAF; AF) is the most common complication after heart surgery.1 Up to 50% of patients who present preoperatively in normal sinus rhythm will develop poAF after cardiac surgery making it the most common postoperative morbidity.2 It is hypothesized that inflammation, increased sympathoadrenal activation, and oxidative stress unmask the propensity for developing poAF in at-risk patients, thereby leading to blood stasis and clot formation in the left atrium.3 Resulting thromboembolism and cerebrovascular accidents can dramatically change the postoperative course and affect patients ’ lives.

Thrombus in transit across a patent foramen ovale in a patient with cerebrovascular accidents, pulmonary embolism, and deep vein thrombosis
We present the case of a 42-year-old woman admitted to the hospital with a third stroke. The presence of a thrombus in transit through a patent foramen ovale (PFO), a deep vein thrombosis (DVT), bilateral pulmonary emboli, and an acute cerebral infarct were concurrently documented.PMID:34269269 | DOI:10.4103/aca.ACA_120_19
