P-019 Comparative effectiveness of stent-assisted coiling and woven endobridge (WEB) embolization for the treatment of unruptured wide-neck bifurcation intracranial aneurysms
ConclusionMid-term complete and adequate occlusion rates were similar between patients treated with WEB and SAC. Given the comparable outcomes, there may be equipoise in treatment options for WNBAs.Abstract P-019 Table 1Baseline patient and aneurysm morphology parameters between treatment groups Aneurysm parameters* WEB (n=27) Stent-coil (n=54) p-value Age 63 (50–69) 65 (57–69) 0.511 Gender, male 6 (22%) 12 (22%) 1.000 Smoking Status Current smoker 9 (33%) 9 (17%) 0.235 Prior Smoker 9 (33%) 22 (41%) Never Smoker 9 (33%) 23 (43%) Location Acomm 9 (33%) 20 (37%) 0.772 Basilar apex 6 (22%) 17 (32%) Carotid termin...

P-032 Underlying intracranial atherosclerotic disease is associated with worse outcomes in acute large vessel occlusion undergoing endovascular therapy
ConclusionUnderlying ICAD is associated with a 2.7-fold increase in the odds of mortality and a 4.5-fold (1/0.22) increase in unfavorable functional outcome at 90 days in patients with LVO undergoing traditional MT. Further research is warranted to understand factors associated with poor outcomes and investigate alternative interventional approaches and medical management in this high-risk population.Disclosures S. O-005 Comparison of on-label versus off-label treatment of intracranial aneurysms with the pipeline embolization devices
ConclusionIn real-world practice, off-label uses of PED can achieve similar safety and efficacy to on-label uses, though there may be a slightly higher rate of ischemic complications in off-label uses. Expert judgment is a useful supplement to official guidelines when assessing reasonable PED use beyond its approved indications.Disclosures S. Cler: None. D. Lauzier: None. A. E-083 Safety and efficacy of xact stent in tandem internal carotid artery and intracranial large vessel occlusion
ConclusionsThe Xact carotid stent is safe and efficacious in the treatment of tandem ICA/ICAO lesions. Larger prospective trials are needed to help confirm our retrospective findings.Disclosures M. Oliver: None. G. Dawod: None. S. Zaidi: None. M. Jumaa: None.

E-039 Comparison of Medical Therapy vs. Intra-Arterial Therapy for Acute Vertebrobasilar Stroke
ConclusionCompared to medical trials in 2015, which demonstrate clinical benefit for IAT over medical therapy for acute ischemic stroke of the anterior circulation, the posterior circulation is less well studied. Important prognostic factors for stroke due to large vessel occlusion of posterior circulation include posterior ASPECTS score and infarct volume in the brainstem. In this small retrospective study, patients treated with IAT had greater stroke severity than the medical cohort, yet they had similar rates of good functional outcome. Parameters of this retrospective analysis may be used to improve triage and treatmen...

64-detector CT angiography within 24 hours after carotid endarterectomy and correlation with postoperative stroke.
CONCLUSIONS CTA within 24 hours of CEA demonstrates characteristic anatomical findings. CCA step-offs and ECA flaps are relatively common and clinically insignificant, whereas ICA/CCA flaps and thrombi are less frequently seen and are associated with postoperative stroke/TIA.
