Paediatric stroke - a review of current guidelines for diagnosis and treatment.
Abstract Stroke, increasingly recognised in children in recent years, is an important cause of long-term morbidity and disability. It can be classified by the stroke type as either arterial ischaemic stroke (AIS), haemorrhagic stroke (HS), or cerebral sinovenous thrombosis (CSVT). Furthermore, perinatal and childhood stroke can be distinguished. A wide range of conditions associated with paediatric stroke has been identified, which differ significantly from those in adults. A paediatric stroke can also present with a variety of symptoms and signs, both specific and non-specific. Because of the diversity of the und...

Age as a prognostic factor in relation to surgical evacuation of spontaneous supratentorial intracerebral haemorrhage.
CONCLUSIONS AND CLINICAL IMPLICATIONS: Neurosurgical treatment of the sICH patients is indicated only if age and potentially improved morbidity is carefully evaluated considering the STICH and this study; otherwise, we will just increase the health care burden with a number of extremely care-dependent patients. Midbrain and bilateral paramedian thalamic stroke due to artery of Percheron occlusion.
CONCLUSION: Bilateral thalamic stroke due to artery of Percheron occlusion is a rare presentation of stroke, which can be overlooked in routine CT scan. If diagnosed, it requires further evaluation for stroke risk factors, especially cardiovascular disorders associated with increased embolic risk. PMID: 27154444 [PubMed - as supplied by publisher]

Reversal of antithrombotic treatment in intracranial hemorrhage--A review of current strategies and guidelines.
Abstract In the last few years, there has been a rapid increase in patients being treated with various anticoagulation and antiplatelet agents. In clinical neurology, these drugs are administered for primary and secondary stroke prevention or to avoid the consequences of immobilization of severe stroke patients. Additionally, thrombolytic intravenous therapy and, recently, intra-arterial therapy for stroke have been increasingly employed all over the world. These therapies are associated with an increased risk of hemorrhage, including the most dangerous, intracranial hemorrhage. The knowledge of the standards for ...

Post-thrombolysis hemorrhage in a patient with hypothyroidism and acute ischemic stroke: Case report.
Abstract Thrombolytic treatment with intravenous recombinant tissue plasminogen activator (rtPA) is an effective treatment for acute ischemic stroke. However, its effectiveness and risks in patients with hypothyroidism have not been reported. Here, we report the case of hemorrhagic transformation after intravenous rtPA thrombolysis treatment in a patient with acute ischemic stroke and hypothyroidism. An apparent edema formed around the hematoma and progressively worsened. He also developed lung infection, electrolyte imbalance, and abnormal liver and kidney functions, and eventually died within 1 month of symptom ...

A patient with acute aortic dissection presenting with bilateral stroke - A rare experience.
Abstract Acute aortic dissection is a rare, life-threatening condition requiring early recognition and proper treatment. Although chest pain remains the most frequent initial symptom, clinical manifestation of aortic dissection varies. Rarely aortic dissection starts with neurological symptoms such as ischemic stroke, which is usually right-sided. A danger of performing thrombolytic therapy in these patients exists if aortic dissection is overlooked. Herein, we present a case of a patient with acute aortic dissection without typical chest pain whose initial manifestation was bilateral stroke. The uncommon presenta...

The rtPA increases MMP-9 activity in serum during ischaemic stroke.
CONCLUSIONS: Thrombolysis applied for IS treatment increases MMP-9 activity in serum, however, rtPA does not facilitate the conversion of pro-MMP-9 into the active form. Our results also suggest the involvement of MMP-3 to the biochemical processes occurring during acute phase of IS. PMID: 25440008 [PubMed - in process]

Combined thrombolysis in posterior circulation stroke caused by bilateral vertebral artery dissection in squash player.
CONCLUSIONS: Vertebral artery dissection should be taken into consideration in differential diagnosis of posterior circulation stroke or TIA in young patients with a history of even as subtle precipitating events as forceful head movements. Combined thrombolytic therapy may provide safe and effective treatment of stroke-complicated cases. This case report shows that expanded diagnostic protocol for acute ischemic stroke, including computed tomography perfusion study and angiography of cervical and cranial vessels, assures rapid and correct diagnosis. Presentation, therapy and outcome of patients with ischemic stroke under new oral anticoagulants.
CONCLUSIONS: Little is known about ischemic strokes under NOAC. To increase the knowledge, the data of 469 ischemic strokes which occurred in NOAC-investigating trials should be analyzed. Furthermore ischemic and bleeding events under NOAC outside clinical trials should be reported. An international registry, independent from the pharmaceutical industry for collecting these informations is desirable. PMID: 24821640 [PubMed - as supplied by publisher]

The efficacy and safety of intravenous thrombolysis with alteplase in the treatment of ischaemic stroke in a rural hospital.
Conclusions: The indications for intravenous thrombolysis in patients with IS should be strictly analysed so that the treatment is effective and safe especially in older patients, patients with greater severity of neurological symptoms and patients with old post-stroke lesions in baseline CT.
