A cardiovascular polypill for secondary stroke prevention in a tertiary centre in Ghana (SMAART): a phase 2 randomised clinical trial
Lancet Glob Health. 2023 Oct;11(10):e1619-e1628. doi: 10.1016/S2214-109X(23)00347-9.ABSTRACTBACKGROUND: A cardiovascular polypill containing generic drugs might facilitate sustained implementation of and adherence to evidence-based treatments, especially in resource-limited settings. However, the impact of a cardiovascular polypill in mitigating atherosclerotic risk among stroke survivors has not been assessed. We aimed to compare a polypill regimen with usual care on carotid intima-media thickness (CIMT) regression after ischaemic stroke.METHODS: In SMAART, a phase 2 parallel, open-label, assessor-masked, randomised clini...

Prevalence of hs-CRP among Indians with hypertension
Bioinformation. 2022 Oct 31;18(10):1041-1043. doi: 10.6026/973206300181041. eCollection 2022.ABSTRACTThe prevalence of hypertension in the early twentieth century varied in India, ranging from 2-15% in Urban India and 2-8% in Rural India. In the inter heart and inter stroke study, hypertension accounted for 17.9% and 34.6% of population attributable risk for coronary artery disease and stroke respectively. CRP appears in serum in response to a variety of inflammatory stimuli .Raised level of hs-CRP is seen with increasing with age, during an infection, inflammation, coronary artery diseases, obesity, sepsis, smoking and va...

Impact of visceral fat on coronary artery disease as defined by quantitative computed tomography angiography
CONCLUSIONS: These findings demonstrate progression with regard to VF and TPV, NCP volume, and LD-NCP volume. Notably, there was a progression of VF and amount of LD-NCP, which is known to be high risk for future cardiovascular events. A consistent progression may indicate the future utility of VF in CVD risk stratification.PMID:37559558 | DOI:10.1002/oby.23804

Peripheral artery disease: Update on etiology, pathophysiology, diagnosis and treatment
Med Clin (Barc). 2023 Jul 28:S0025-7753(23)00361-5. doi: 10.1016/j.medcli.2023.06.005. Online ahead of print.ABSTRACTPeripheral artery disease (PAD) is a condition related to atherosclerosis affecting >200 million people worldwide, and it increases cardiovascular morbidity (mainly from myocardial infarction and stroke) and mortality. Indeed, PAD patients are classified as patients at very high cardiovascular risk. The most common manifestation of PAD is intermittent claudication, which is associated with reduced mobility and leg pain. Nevertheless, asymptomatic PAD is the most frequent form of PAD worldwide; therefore, ...

Do Non-Functional Adrenal Adenomas Affect Metabolic Profile and Carotid Intima-Media Thickness? A Single Centre Study from Poland
Conclusions: Patients diagnosed with NFAIs, despite normal cortisol inhibition in the 1 mg dexamethasone test, still presented a number of metabolic abnormalities. The assessment of IMT may proove valuable in indicate the presence of early vascular remodelling in asymptomatic patients. The underlying mechanisms of these findings are still unknown, hence further studies are required.PMID:37510727 | PMC:PMC10380859 | DOI:10.3390/jcm12144612

Long-term effectiveness, outcomes and complications of bariatric surgery
World J Clin Cases. 2023 Jul 6;11(19):4504-4512. doi: 10.12998/wjcc.v11.i19.4504.ABSTRACTDietary imbalance and overeating can lead to an increasingly widespread disease - obesity. Aesthetic considerations aside, obesity is defined as an excess of adipose tissue that can lead to serious health problems and can predispose to a number of pathological changes and clinical diseases, including diabetes; hypertension; atherosclerosis; coronary artery disease and stroke; obstructive sleep apnea; depression; weight-related arthropathies and endometrial and breast cancer. A body weight 20% above ideal for age, gender and height is a...

ICAM-1 and VCAM-1: gatekeepers in various inflammatory and cardiovascular disorders
Clin Chim Acta. 2023 Jul 11:117487. doi: 10.1016/j.cca.2023.117487. Online ahead of print.ABSTRACTLeukocyte migration from the vascular compartment is critical fornormal lymphocyte recirculation in specific tissues and immune response in inflammatory locations. Leukocyte recruitment, migration to inflammatory areas, and targeting in the extravascular space are caused by cellular stimulation and local expression of adhesion molecules. Intercellular adhesion molecule 1 (ICAM-1) and Vascular cell adhesion molecule 1 (VCAM-1) belong to the immunoglobulin superfamily of cell adhesion molecules with a crucial role in mediating t...

Insulin Regulation of Hepatic Lipid Homeostasis
Compr Physiol. 2023 Jun 26;13(3):4785-4809. doi: 10.1002/cphy.c220015.ABSTRACTThe incidence of obesity, insulin resistance, and type II diabetes (T2DM) continues to rise worldwide. The liver is a central insulin-responsive metabolic organ that governs whole-body metabolic homeostasis. Therefore, defining the mechanisms underlying insulin action in the liver is essential to our understanding of the pathogenesis of insulin resistance. During periods of fasting, the liver catabolizes fatty acids and stored glycogen to meet the metabolic demands of the body. In postprandial conditions, insulin signals to the liver to store exc...

Endocan is Related to Increased Cardiovascular Risk in Type 2 Diabetes Mellitus Patients
Conclusion: Endocan was the independent predictor for moderate and high estimated risks (i.e., nonfatal and fatal CHD and nonfatal stroke risk scores) in T2D patients. When included in models with sex and obesity indices endocan demonstrated good clinical accuracy in discriminating T2D patients with high risk for nonfatal and fatal eCHD and nonfatal stroke risk from those patients with low risk.PMID:37279462 | DOI:10.1089/met.2023.0050

Age at diagnosis, diabetes duration and the risk of cardiovascular disease in patients with diabetes mellitus: a cross-sectional study
CONCLUSION: Age at diagnosis, diabetes duration, hypertension and hyperlipidemia were independent risks of CVD. Longest (>15 years) diabetes duration increased the high risk of ten-year ASCVD prediction among Chinese patients with DM. It's urgent to emphasize the importance of age at diagnosis and diabetes duration to improve primary complication of diabetes.PMID:37223032 | PMC:PMC10200881 | DOI:10.3389/fendo.2023.1131395

Association of visceral adiposity index with asymptomatic intracranial arterial stenosis: a population-based study in Shandong, China
CONCLUSIONS: The possible correlation between VAI and aICAS was found to be positive for the first time among Chinese rural residents over 40 years old. A higher VAI was found to be significantly associated with aICAS among the participants who were underweight or normal weight, and these results may provide additional risk stratification information for aICAS.PMID:37198613 | DOI:10.1186/s12944-023-01831-1
