SMART Syndrome (Stroke-like migraine attacks after radiation therapy): When to suspect it?

Ovarian Cancer Presenting as Cryptogenic Stroke from Patent Foramen Ovale.
Abstract A 52-year-old woman experienced severe headache, confusion, nausea, dizziness, and diplopia for three days. Magnetic resonance imaging of the brain showed multiple acute and subacute infarcts suggestive of embolic events. Dermatological examination was notable for splinter hemorrhages and macular patches on the fingernails and feet, respectively. Further diagnostic imaging of the chest and abdomen revealed pulmonary emboli and an ovarian mass with omental deposits and splenic infarcts. A Case of Posterior Reversible Encephalopathy Syndrome Induced by Cisplatin/Pemetrexed Chemotherapy for Lung Cancer
This report presents the case of a 60-year-old woman who was diagnosed with stage IV lung adenocarcinoma with asymptomatic brain metastases and commenced chemotherapy with cisplatin/pemetrexed (CDDP/Pem). She experienced tonic-clonic convulsions on day 9 of the first cycle, which were accompanied by increased blood pressure (173/69 mm Hg) and headache. Pointing to the diagnosis: hemorrhagic brain metastasis
The patient ’s symptoms and initial imaging results directed the original diagnosis towards hemorrhagic stroke, but this conclusion proved incorrect. A 65-year-old man presented with a 1-week history of dull headache, nausea, and vomiting. His medical history was significant for long-term hypertension and sta ge 1B lung adenocarcinoma. Right Hemispheric Acute Ischemic Stroke Secondary To Right Common Carotid Artery Free Floating Thrombus Due To Hypercoagulable State In An Otherwise Healthy 48-Year-Old Woman. (P6.263)
Conclusions: Due to the high association between malignancy and hypercoagulable state, further work up needs to be warranted in the evaluation of a patient with FFT. To our knowledge this is the first case report of symptomatic FFT treated with an oral direct factor-Xa inhibitor.
