Transcatheter and Surgical Aortic Valve Replacement in Patients with Recent Acute Heart Failure.
CONCLUSIONS: Recent AHF is associated with increased risk of mortality and morbidity after SAVR and TAVR. These findings suggest that AS patients should be referred for invasive treatment before the development of clinically evident heart failure. PMID: 31288017 [PubMed - as supplied by publisher]

Long-term outcomes with mechanical versus biological aortic valve prosthesis in patients > 70 years.
CONCLUSIONS: Mechanical aortic valve prosthesis is associated with worse long-term survival and increased bleeding after SAVR in patients aged >70 years. Our results suggest caution when considering mechanical aortic valve prostheses in elderly patients. PMID: 31082357 [PubMed - as supplied by publisher]

The Impact of Preoperative Clopidogrel on Outcomes Following Coronary Artery Bypass Grafting.
CONCLUSIONS: CABG can be safely performed in patients with recent Clopidogrel exposure with no increased risk of early or late mortality. However, CABG should be delayed for at least 5 days if clinically feasible to minimize transfusions and reoperation risk. In patients with more urgent indications, delaying CABG at least 3 days can mitigate but not eliminate the increased bleeding risk. PMID: 31039353 [PubMed - as supplied by publisher]

Impact of Bleeding Revision on Outcomes after Left-Ventricular Assist Device Implantation.
CONCLUSIONS: A bleeding revision is associated with significantly reduced survival and substantially increased morbidity during LVAD support. Pre-, intra- and post-operative prophylactic strategies may help to prevent this life-threatening complication. PMID: 30851258 [PubMed - as supplied by publisher]

The Society of Thoracic Surgeons Intermacs Database Annual Report: Evolving Indications, Outcomes, and Scientific Partnerships.
CONCLUSIONS: With the evolution of MCS, patient phenotype and outcomes are also changing over time. CF LVAD support is increasingly being used in the less ill patient phenotype and more patients are supported for destination therapy. Mean survival is now approaching 5 years, but adverse events, especially neurologic events, continue to have a detrimental impact on the success of CF LVAD support. PMID: 30691584 [PubMed - in process]

Complete sternal-sparing HeartMate 3 implantation: a case series of ten consecutive patients.
CONCLUSIONS: Our early outcomes suggest that the CSS approach may reduce the incidence of right ventricular failure, bleeding, intubation time and ICU length of stay. Further studies are needed to fully elucidate the advantages of CSS LVAD implantation compared with median sternotomy. PMID: 30444989 [PubMed - as supplied by publisher]

Factors Associated with Prolonged Survival in Left Ventricular Assist Device Recipients.
CONCLUSIONS: Preoperative diabetes and amiodarone use were associated with poor long-term survival in LVAD recipients. Development of early complications of stroke, gastrointestinal bleeding, hemolysis, or pump thrombosis was also associated with poor long-term survival. Early diagnosis and treatment of these complications may improve survival in LVAD recipients. PMID: 30316851 [PubMed - as supplied by publisher]

Feasibility and Outcomes of TAVI using the Left Axillary Artery as Primary Access Site.
CONCLUSIONS: This is the first study reporting on outcome after TAVI using the LAA as default access. We conclude that it is highly feasible and safe with low rates of major vascular complications, bleeding and stroke. PMID: 30292844 [PubMed - as supplied by publisher]

Impact of Frailty on Outcomes in Acute Type A Aortic Dissection.
CONCLUSIONS: Frailty, as defined using a 7-component frailty index, can serve as an independent predictor of the risk of late mortality in patients undergoing an ATAAD. Such frailty markers, all of which are easily assessed preoperatively, may provide valuable information for patient counseling and risk stratification before aortic surgery. Valvectomy versus Replacement for the Surgical Treatment of Tricuspid Endocarditis.
CONCLUSIONS: Tricuspid valve excision, repair, and replacement have similar 30-day operative mortality, as defined by the STS, in TV endocarditis patients and significantly lower unplanned readmission rates at one year in the excision patients. Tricuspid valvectomy is an acceptable initial treatment in this high-risk group as part of a surgical strategy to identify patients who are candidates for eventual valve replacement. Further study of long term outcomes and survival is warranted. PMID: 29777672 [PubMed - as supplied by publisher]

Hybrid Coronary Revascularization Versus On-Pump Coronary Artery Bypass Grafting.
CONCLUSIONS: For selected patients, HCR is associated with a faster postoperative recovery as well as similar short-term and long-term outcomes when compared with standard on-pump CABG. PMID: 29571730 [PubMed - as supplied by publisher]

Coronary Revascularization with Single Versus Bilateral Mammary Arteries: Is It Time to Change?
CONCLUSIONS: In a large regional experience BIMA is associated with no upfront risk of adverse events and improved long-term survival compared to SIMA. Our results indicate that BIMA conduits should be considered more frequently during CABG due to their demonstrated survival advantage. PMID: 29550203 [PubMed - as supplied by publisher]

Assessment of Commonly Used Frailty Markers for High- and Extreme-Risk Patients Undergoing Transcatheter Aortic Valve Replacement.
CONCLUSIONS: Among high- and extreme-risk patients undergoing TAVR, our new frailty model was more discriminative of 30-day mortality than The Society of Thoracic Surgeons predicted risk of mortality. New cutoff values for frailty indicators were identified and will require further validation. PMID: 28942076 [PubMed - indexed for MEDLINE]
