Breast CA antihormones, concurrent vs sequential
I have a patient with very poor health who cannot medically undergo surgery - heart failure, O2 dependent - but we are treating a bulky breast mass about 7 cm in size and grossly involved regional nodes. CHF treatment options
Why are calcium channel blockers (CCB) contra-indicated in congestive heart failure, while beta-blockers aren't? I understand that CCB may worsen the situation with a significant drop in cardiac output (CO), since they're negative inotropes and can further deteriorate the cardiac function.... but don't beta-blockers ultimately do the same thing? They're negative chronotropes, so they will also cause a decrease in CO. Why can they be used in heart failure while CCB cannot? Thanks in advance. (Source: Student Doctor Network)

Loved the medicine and knowledge on IM, kind of hated the day to day
MS2 in my clinical year trying to figure out what I want to do with my life. Just got done my inpatient IM rotation and I had some mixed thoughts. I went in thinking I was going to like it, but I’m conflicted. Attitude change with younger RTs
Anyone else find themselves working with a lot of gen Z/young millennial RTs that like to have opinions? I'm locums working at multiple sites and the RTs keep trending younger, the game now is apparently to talk the physician down from whatever intervention is requested. Patient needs a tube? Why not put them on bipap instead. Patient needs bipap? Why not put them on a nonrebreather for heart failure. Need deep suction for bronchiolitis? Why not try a bulb. Young people suck. Decongestion and acetazolamide
Really interesting trial from European Society of Cardiology. ADOVAR. Adding acetazolamide 500mg IV daily + the usual IV frusemide resulted in higher rates of clinical decongestion at 3 days with no signals of harm in acute decompensated heart failure. I suspect this practice will become more widely adopted, even for garden variety de-resuscitation.
