How to do better on clinical reasoning for osce?
I did not perform too well on my school’s osce so the faculty wants me to do it again. It looks like I did well on interviewing and physical exams but I came up with wrong diagnoses/differentials based on what I have learned from my standardized patients during the interview. Would you do this case? 90yo patient for a hip ORIF. Dementia, CKD3, DM, COPD, prior mitral valve repair, echo from a year ago showing moderate-severe AR and moderate TR, there is a dilated RV and LV, LVEF35%, pHTN with PA pressures up to 50. Advanced CHF docs?
I have noticed that the cardiologists in my location do not seem very good at managing critically ill patients in heart failure (eg cardiogenic shock, end stage CHF). I see tiny doses of lasix, no comments on inotrope or pressor support strategies, no mention of ECMO. An interesting case
52 yo male scheduled for right thoracoscopy for tumor vs empiema (tough surgery was sure It was on the cancer side) with a history of heavy smoking over 20 years, HBP with no treatment, dilated miocardiopath, heart failure with FEV1 20%, recent onset AF. Current meds are digoxine, furosemide, enalapril and metoprolol.
