Image-guided glucocorticoid injection versus injection without image guidance for shoulder pain
CONCLUSIONS: Our updated review does not support use of image guidance for injections in the shoulder. Moderate-certainty evidence indicates that ultrasound-guided injection in the treatment of shoulder pain probably provides little or no benefit over injection without imaging in terms of pain or function and low-certainty evidence indicates there may be no difference in quality of life. We are uncertain if ultrasound-guided injection improves participant-rated treatment success, due to very low-certainty evidence. Low-certainty evidence also suggests ultrasound-guided injection may not reduce the risk of adverse events co...

Pharmacokinetics and pharmacodynamics of sildenafil in fetal lambs on extracorporeal support
Biomed Pharmacother. 2021 Sep 16;143:112161. doi: 10.1016/j.biopha.2021.112161. Online ahead of print.ABSTRACTBACKGROUND: Maternal transplacental administration of sildenafil is being considered for a variety of fetal conditions. Clinical translation also requires evaluation of fetal safety in a higher species, such as the fetal lamb. Experiments with the pregnant ewe are curtailed by minimal transplacental transfer as well as limited access to the fetus. The EXTra-uterine Environment for Neonatal Development (EXTEND) model renders the isolated fetal lamb readily accessible and allows for direct fetal administration of sil...

Imaging modalities for the detection of posterior pelvic floor disorders in women with obstructed defaecation syndrome
CONCLUSIONS: In a population of women with symptoms of ODS, none of the imaging techniques met the criteria to replace EP. MRI and TPUS met the criteria of a triage test, as a positive test confirms diagnosis of rectocele, enterocele and intussusception, and a negative test rules out diagnosis of anismus. An evacuation phase increased sensitivity of MRI. Rectal contrast did not increase sensitivity of TPUS. QoE of EVUS, DAE and EDF was too low to draw conclusions. More well-designed studies are required to define their role in the diagnostic pathway of ODS.PMID:34553773 | DOI:10.1002/14651858.CD011482.pub2

Postnatal outcome in patients with aortic stenosis undergoing fetal aortic valvuloplasty: systematic review and meta-analysis
CONCLUSION: This study showed a BVC rate of 46% among live-born patients with AS undergoing FAV. Bias might be introduced due to the lack of randomized clinical trials and BVC rate improved to 52% when subjects underwent a technically successful FAV. However, currently, data does not suggest a true benefit after FAV towards BVC. This article is protected by copyright. All rights reserved.PMID:34726817 | DOI:10.1002/uog.24807

Ultrasound-guided transvaginal ovarian needle drilling for clomiphene-resistant polycystic ovarian syndrome in subfertile women
CONCLUSIONS: Since we did not identify any studies for inclusion, we were unable to assess the benefit or harm of applying UTND for women with clomiphene-resistant PCOS.PMID:34735019 | DOI:10.1002/14651858.CD008583.pub3

Diagnosis of placenta accreta spectrum in high-risk women using ultrasonography or magnetic resonance imaging: systematic review to compare accuracy of tests
CONCLUSION: US and MRI have similar accuracy characteristics for PAS diagnosis. These results suggest that, in settings with high prevalence of risk factors, the screening for PAS should start with the most available test or the one which the providers are more apt to interpret.
