Moral distress amongst palliative care doctors working during the COVID-19 pandemic: A narrative-focussed interview study
CONCLUSIONS: Restrictions had a considerable impact on palliative care doctors' ability to communicate with and comfort patients which led to moral distress and contributed to decreasing morale. Future research could explore moral distress in palliative care settings internationally during the pandemic with a view to compare the factors affecting how moral distress was experienced.PMID:35548937 | DOI:10.1177/02692163221088930

809 Survey of current and former academic clinical fellows in emergency medicine in the UK
Conclusion We found variation in how the schemes are implemented. Future research is required to explore the experiences of those undertaking EM ACFs in more detail to identify the features of successful ACF training schemes, and whether any aspects may benefit from standardisation.

Pediatric Celiac Disease and Eosinophilic Esophagitis: Outcome of Dietary Therapy
Conclusions: To our knowledge, this is the largest pediatric study to assess the histologic outcome of EoE-associated esophageal eosinophilia in response to dietary management of pediatric patients with concomitant CeD and EoE. We demonstrate that soy is well tolerated in this cohort, and suggest that reintroducing this food first, or trialing a soy-inclusive elimination diet is a viable strategy.

Conclusion: KMT2A-rearranged MMECs in ALL patients may have inherently more aggressive behavior, even when the histomorphology of MMEC suggests a low-grade malignancy.PMID:37517063 | DOI:10.1080/15513815.2023.2241903

Nutrition in the ICU: new trends versus old-fashioned standard enteral feeding?
Purpose of review The narrative review aims to summarize the relevant studies from the last 2 years and provide contextual information to understand findings. Recent findings Recent ICU studies have provided insight in the pathophysiology and time course of catabolism, anabolic resistance, and metabolic and endocrine derangements interacting with the provision of calories and proteins. Early provision of high protein intake and caloric overfeeding may confer harm. Refeeding syndrome warrants caloric restriction and to identify patients at risk phosphate monitoring is mandatory. Infectious complications of parenteral...

You Asked: What Is Inflammation, And Why Should I Care About It?
Your body can heal itself, which is pretty miraculous when you sit back and think about it. If you suffer a cut or infection—or if a disease, allergen or virus finds its way into you—your immune system reacts by sending specialized white blood cells to the affected area. These white blood cells can repair damage, stop the spread of infection or illness and in some cases eradicate the intruder. This whole response is called inflammation. “Inflammation is an activation of cells and cell-derived components that have the job of fighting invasions, and in some cases just sponging up or clearing out damaged cel...

Choosing life with a VAD (ventricular assist device)
Rain splattered, blurring my view of the Massachusetts state highway. The rental car’s wipers squeaked as they dragged across the windshield. Though I was briefly tempted to turn back, I kept driving. The man with the battery-operated heart had invited me to his home, and I didn’t want to be late. I am a critical care doctor. Throughout the course of my training, I have learned how to manage a ventilator, how to treat sepsis, how to sort out the causes of renal failure. But what I didn’t learn is what comes after for those who do not die, whose lives are extended by days, months, or even years as a result of our cutt...
