Right heart failure after left ventricular assist device implantation – from prediction to action
In this issue of the Journal, Read and colleagues1 report the use of peak stroke volume index (SVI) during pre-procedure vasodilatory testing as a predictor of early right heart failure (RHF) after left ventricular assist device (LVAD) implantation. They retrospectively examine 70 patients from three institutions who underwent pre-implant nitroprusside vasodilator testing. Indications for pre-operative vasodilator testing were elevated pulmonary vascular resistance, pulmonary arterial wedge pressure, or systolic pulmonary artery pressure.

Three-year experience of catheter-based micro-axial left ventricular assist device, Impella, in Japanese patients: the first interim analysis of Japan registry for percutaneous ventricular assist device (J-PVAD)
AbstractCatheter-based micro-axial ventricular assist device Impella ® (Abiomed, Danvers, MA) has been used in Japanese patients with drug-refractory acute heart failure (AHF) since 2017. This is the first interim analysis of the ongoing Japan Registry for Percutaneous Ventricular Assist Device (J-PVAD) to investigate the safety and efficacy of Impella support. Betw een October 2017 and January 2020, 823 Japanese patients, who were treated with the Impella 2.5, CP, or 5.0 pump, were enrolled. The primary endpoints were safety profiles and cumulative 30-day survival. Among them, 44.8% of patients were acute myocardial infa...

Comparing Velocity and Mean Arterial Pressure to Assess Pulsatility in Patients with Continuous Flow Left Ventricular Assist Devices
An increasing number of end-stage heart failure patients are supported by continuous-flow Left Ventricular Assist Devices (cfLVADs). In these patients, accurate assessment of arterial pulsatility remains a challenge because of diminished pulse pressure. Limited pulsatility in modern cfLVADs itself can be associated with adverse events such as stroke. Our study investigates the value of simultaneous arterial pressure and arterial blood velocity assessment in cfLVAD patients.

Optimization of Left Ventricle Size After VAD Implantation Prevents Incidence of Cerebral Infarction
Stroke after ventricular assist device (VAD) implantation is a critical complication. Previous studies have noted the distance between the inflow cannula and the left ventricular (LV) septum, as well as an angulation of the inflow cannula from the apical axis, related to the incidence of stroke. In the early phase after VAD implantation, diuresis is caused by improved renal blood flow, furthermore, rapid volume reduction of LV greatly changes circulating plasma volume, resulting in increased stroke risk; however, little is known about an appropriate indicator for the prevention of this phenomenon.

Postoperative Atrial Fibrillation in Lung Transplant Recipients with and without Concomitant Modified Maze
Atrial fibrillation is common following lung transplantation and may contribute to increased risk of stroke. The goal of this study was to characterize the incidence of atrial fibrillation in lung transplant recipients with and without a concomitant modified Maze procedure.

Unique Findings of Myocardial Improvement with Lower Left Ventricular Assist Device Pump Speed
Currently, optimizing hemodynamics and unloading in patients with left ventricular assist device (LVAD) relies on echocardiography and right heart catherization guidance for pump speed adjustments. However, the ideal unloading strategy to maximize reverse remodelling is uncertain - whether to provide maximal pump support to unload the heart ( ‘rest’), or maximize ventricular work with partial pump support (‘work’). Using non-invasively derived PV loops, we sought to characterize the longitudinal changes in chamber contractile parameters stroke work (SW), peak systolic pressure (LVPmax) and peak LV elastance (Ees) a...

A Prospective Randomized Trial Comparing Stroke Volume Measurements by Esophageal Doppler and Pulse-Contour Analysis During a Fluid Resuscitation Protocol in Brain-Dead Organ Donors
Maximizing stroke volume (SV) is the optimal method of fluid resuscitation. Both the esophageal doppler (EDM) and pulse contour analysis (PCA) are validated methods of measuring SV in critical patients, but neither have not been well-studied in the brain-dead (BD) donor. We have previously published our fluid resuscitation protocol in BD donors demonstrating a significant reduction in time on vasopressors from 16 hours in the control group to 2.9 hours in the protocol group. We performed a prospective randomized trial comparing the effectiveness of the EDM and PCA in this fluid resuscitation protocol in BD donors.

Sex-Based Differences in Clinical Outcomes of Stroke Related to LVAD
Sex-based differences among patients with left ventricular assist devices (LVAD) having stroke are uncertain. We aimed to compare demographic and clinical characteristics, inpatient outcomes, and measures of health care utilization among men and women who had stroke while undergoing LVAD implantation.

Who is at Risk for Seizures After Heart Transplantation?
Seizures may occur following heart transplantation (HTx), especially since tacrolimus can lower a seizure threshold. HTx recipients are also at high stroke risk given many patients have existing cerebrovascular disease. The purpose of the study was to assess the prevalence and risk factors for seizures in HTx recipients at our center.

Carotid Endarterectomy Prior to Ventricular Assist Device Placement in a Patient with Cardiogenic Shock
Atherosclerotic disease in the cervical vessels may be a contraindication for implantation of left ventricular assist devices (LVADs). The occurrence of ischemic stroke represents an obstacle to rehabilitation, and its incidence seems to be higher in this population. Carotid endarterectomy (CEA) is a preferable surgical treatment in patients with high bleeding risk. This case report suggests that CEA followed by implantation of LVADs can be feasible.

Assessing Cardiac Recovery for Decommissioning of LVAD Support: Structure versus Function
Recovery of left ventricular (LV) function during LVAD support occurs in only 1-3% of patients. Assessment for recovery and LVAD removal is desirable given the risks of infection, stroke, and device malfunction. This case of a patient with an LVAD, with recovered LV systolic function but abnormal exercise hemodynamics, illustrates the potential utility of right heart catheterization studies for clinical decisions.
