Sex Differences in Intracranial Atherosclerosis in Patients With Hypertension With Acute Ischemic Stroke
Conclusions Men with hypertension with acute ischemic stroke have significantly higher total and proximal plaque burdens than women. Women with hypertension on anti-hypertensive medication showed a greater reduction in proximal plaque burden than men. Further confirmation with a longitudinal cohort study is needed and may help evaluate whether different treatment guidelines for managing hypertension by sex can help reduce intracranial atherosclerosis burden and ultimately acute ischemic stroke risk.PMID:35574965 | DOI:10.1161/JAHA.122.025579

Comparison of Dabigatran Versus Warfarin Treatment for Prevention of New Cerebral Lesions in Valvular Atrial Fibrillation
Warfarin is the standard anticoagulation therapy for valvular atrial fibrillation (AF); however, new oral anticoagulants have emerged as an alternative. We compared the efficacy and safety of dabigatran with conventional treatment in AF associated with left-sided valvular heart disease (VHD), including mitral stenosis (MS). Patients with AF and left-sided VHD were randomly assigned to receive dabigatran or conventional treatment. The primary end point was the occurrence of clinical stroke or a new brain lesion (silent brain infarct and microbleed) on 1-year follow-up brain magnetic resonance imaging.

Left Atrial Remodeling and Stroke in Patients With Sinus Rhythm and Normal Ejection Fraction: ARIC-NCS
Conclusions In a large cohort study of elderly patients, among participants with sinus rhythm, normal ejection fraction, and no history of atrial fibrillation, measures of worse age-related LA reservoir function and stiffness are associated with higher odds of SCIs and stroke.PMID:35491988 | DOI:10.1161/JAHA.121.024292

Acute Cerebral Microinfarcts in Acute Ischemic Stroke: Imaging and Clinical Significance
CONCLUSIONS: We have shown A-CMIs occur in cortical and subcortical regions in nearly half of AIS patients, where microembolism and CSVD are, respectively, the key presumed etiological mechanism.PMID:35490673 | DOI:10.1159/000524021

Fabrication of Customizable Intraplaque Hemorrhage Phantoms for Magnetic Resonance Imaging
CONCLUSIONS: The cylindrical IPH phantom accurately modeled the short T1 time characteristic of methemoglobin-laden IPH, with the IPH sites exhibiting little variation in imaging properties over 31 days. Furthermore, MPRAGE images of the anatomical atherosclerosis replicas closely matched those of clinical plaques. In combination, these phantoms will allow for IPH imaging protocol standardization and thus facilitate future multicenter IPH imaging.PMID:35486294 | DOI:10.1007/s11307-022-01722-4

Comparison of carotid atherosclerotic plaque characteristics between symptomatic patients with transient ischemic attack and stroke using high-resolution magnetic resonance imaging
CONCLUSIONS: Ischemic stroke patients had larger plaque burden and greater proportion of LRNC in carotid plaques compared to those with TIA. This study suggests that ischemic stroke patients had more vulnerable plaques compared to those with TIA.PMID:35448952 | DOI:10.1186/s12872-022-02624-7

Reclassification and risk stratification of embolic stroke of undetermined source by ASCOD phenotyping
Conclusions:ASCOD grade A2 or A3 was predictive of MACE in ESUS patients. Reclassification of ESUS using ASCOD phenotyping provides important clues for risk prediction and may guide optimal management strategies.PMID:35422186 | DOI:10.1177/17474930221096953

Effect of Mid-Basilar Artery Angle and Plaque Characteristics on Pontine Infarction in Patients with Basilar Artery Plaque
CONCLUSION: The mid-BA angle may increase the incidence of pontine infarction by increasing the plaque burden.PMID:35418542 | DOI:10.5551/jat.63520
