Research Articles Ventricular stroke work and vascular impedance refine the characterization of patients with aortic stenosis
Aortic stenosis (AS) management is classically guided by symptoms and valvular metrics. However, the natural history of AS is dictated by coupling of the left ventricle, aortic valve, and vascular system. We investigated whether metrics of ventricular and vascular state add to the appreciation of AS state above valve gradient alone. Seventy patients with severe symptomatic AS were prospectively followed from baseline to 30 days after transcatheter aortic valve replacement (TAVR). Quality of life (QOL) was assessed using the Kansas City Cardiomyopathy Questionnaire. Left ventricular stroke work (SWLV) and vascular impedance...

Cardiac papillary fibroelastomas: A 10-year single-center surgical experience and long-term echocardiographic follow-up study.
CONCLUSION: CPF can be safely and effectively treated surgically. TEE is superior to the TTE option in CPF post-operative recurrence detection. There is a clear need for a prospective study to determine criteria for embolization risk stratification and optimum management in patients with CPF. Transcatheter aortic valve implantation.
Authors: Oliemy A, Al-Attar N Abstract Transcatheter aortic valve implantation was developed to offer a therapeutic solution to patients with severe symptomatic aortic stenosis who are not candidates for conventional aortic valve replacement. The improvement in transcatheter aortic valve implantation outcomes is still of concern in the areas of stroke, vascular injury, heart block, paravalvular regurgitation and valve durability. Concomitantly, the progress, both technical and in terms of material advances of transcatheter valve systems, as well as in patient selection, renders transcatheter aortic valve implantati...

CHADS2 and CHA2DS2-VASc Scoring Systems for Predicting Atrial Fibrillation following Cardiac Valve Surgery
by Liang Yin, Xinyu Ling, Yufeng Zhang, Hua Shen, Jie Min, Wang Xi, Jing Wang, Zhinong Wang Objective Clinical use of CHADS2 and CHA2DS2-VASc scoring systems for predicting AF following cardiac surgery have been reported in previous studies and demonstrated well-validated predictive value. We sought to investigate whether the two scoring systems are effective for predicting new-onset of AF following cardiac valve surgery and to demonstrate its potential utility of clinical assessment. Methods Medical records of all patients underwent cardiac valve surgeries during the period of January 2003 and December 2013 without preope...

Assistive technology using regurgitation fraction and fractional-order integration to assess pulmonary valve insufficiency for pre-surgery decision making and post-surgery outcome evaluation
This study proposes an assistive technology to quantify regurgitation using the regurgitation fraction (RF) and heart pump efficiency (HPE). In signal preprocessing stage, the detrending and zero-crossing processes are used to remove the unwanted flow fluctuations and identify the end-systolic and end-diastolic periods per each cardiac cycle. The fractional-order integrations are employed to calculate the stroke volume (SV) and regurgitation volume (RV). Then, the regurgitation flow can be quantified that indicates the high correlation with HPE. For a mimicking pulmonary circulation loop system, the proposed screening mode...

Early and Two-year Outcomes after Sutureless and Conventional Aortic Valve Replacement: a Nationwide Population-based Study
CONCLUSION: Early and 2-year clinical outcomes between SAVR and CAVR were not different except for a high rate of permanent pacemaker implantation in the SAVR group.PMID:33686809 | DOI:10.3346/jkms.2021.36.e57

Sociodemographic disparities in concomitant left atrial appendage occlusion during cardiac valve operations
by Ayesha P. Ng, Nikhil Chervu, Yas Sanaiha, Amulya Vadlakonda, Elsa Kronen, Peyman Benharash BackgroundSociodemographic disparities in atrial fibrillation (AF) management and thromboembolic prophylaxis have previously been reported, which may involve inequitable access to left atrial appendage occlusion (LAAO) during cardiac surgery. The present study aimed to evaluate the association of LAAO utilization with sex, race, and hospital region among patients with AF undergoing heart valve operations. MethodsAdults with AF undergoing valve replacement/repair in the 2012 –2019 National Inpatient Sample were identified and str...
