Use of the ClearSight ® System for Continuous Noninvasive Hemodynamic Monitoring during Heart Valve Interventions: Review of the Literature and Single-Site Experience.
Use of the ClearSight® System for Continuous Noninvasive Hemodynamic Monitoring during Heart Valve Interventions: Review of the Literature and Single-Site Experience. Heart Surg Forum. 2018 Nov 14;21(6):E476-E483 Authors: Gellert G, Bramlage P Abstract During interventional and structural cardiology procedures, such as mitral valve (MitraClip, BMV), aortic valve (TAVR, BAV), tricuspid valve (MitraClip), left atrial appendage (Watchman, Lariat), atrial septum (ASD/PFO closure), and coronary artery intervention (high-risk PCI), among others, patients are at a high risk of hemodynamic instability and re...

Initiation of ivabradine in cardiogenic shock
ConclusionsIn our small non ‐randomized series of patients in cardiogenic shock, ivabradine was safely used to reduce HR in patients previously intolerant of beta‐blockade. There are limited data surrounding the use of ivabradine in cardiogenic shock, and future studies should be undertaken to determine the optimal HR in h umans with cardiogenic shock and whether systematic limitation of peak HR may improve outcomes.

Early Hemodynamic Changes and Long-Term Outcome Of Patients With Severe Low Gradient Aortic Stenosis After Transcatheter Aortic Valve Replacement.
CONCLUSIONS: NFLG-AS was the most prevalent form of LG severe AS and was associated with adequate left ventricular compensation and good prognosis. On the other hand, CLFLG-AS represents the heart-failure with reduced ejection fraction (HFrEF) form of AS and was associated with the worst prognosis, whereas PLFLG-AS represents the heart-failure with preserved ejection fraction (HFpEF) form of AS with intermediary prognosis. Both groups showed early hemodynamic reverse response after TAVR. PMID: 31566569 [PubMed - as supplied by publisher]

Vena cava backflow and right ventricular stiffness in pulmonary arterial hypertension
Vena cava backflow is a well-recognised clinical hallmark of right ventricular failure in pulmonary arterial hypertension (PAH). Backflow may result from tricuspid regurgitation during right ventricular systole or from impaired right ventricular diastolic filling during atrial contraction. Our aim was to quantify the forward and backward flow in the vena cava and to establish the main cause in PAH. In 62 PAH patients, cardiac magnetic resonance measurements provided volumetric flows (mL·s–1) in the superior and inferior vena cava; time integration of flow gave volume. The "backward fraction" was defined as the...

Timeliness of Care for High Acuity Conditions at Hospital-Affiliated Freestanding Emergency Departments.
CONCLUSIONS: FREDs provided more timely care than HEDs for visits with asthma exacerbation and less timely care for acute chest pain, stroke, and sepsis, although absolute differences were small. Even though STEMI patients at FREDs required transfer for catheterization, they tended to receive care in line with national guidelines. PMID: 32352204 [PubMed - as supplied by publisher]

Plasma proteoglycan prolargin in diagnosis and differentiation of pulmonary arterial hypertension
ConclusionsPlasma prolargin levels differentiate PAH patients from controls and the other investigated dyspnoea groups including HF. Its potential in PAH differentiation may be enhanced by inclusion in a multi ‐marker panel. Larger studies are needed to evaluate its discriminative ability of PAH in relation to other dyspnoea aetiologies and its potential role in PAH risk stratification and pathobiology.

Outcomes of Patients Undergoing Cardiac Catheterization After a Three-Day Holiday Weekend Versus a Two-Day Weekend
CONCLUSIONS: Cardiac catheterization procedures performed after a 3-day weekend were not associated with differences in in-patient mortality, myocardial infarction, stroke, or procedural complications.PMID:34740172
