Less-Invasive Aortic Valve Replacement: Trends and Outcomes from the STS Database.
CONCLUSIONS: LI-AVR is associated with a similar operative mortality and postoperative stroke rate compared to FS. LI-AVRS should be served as a benchmark for comparison between TAVR and sAVR in low risk patients. PMID: 32835750 [PubMed - as supplied by publisher]

Simultaneous hybrid carotid stenting and coronary bypass surgery versus concomitant open carotid and coronary bypass surgery: a pilot, feasibility study ADULT CARDIAC
CONCLUSIONS Although limited by a small sample size, our results show that the hybrid procedure of carotid stenting and coronary surgery might be a good therapeutic option but further extended studies are needed to assess its real value.

Analysis of Autologous Platelet-Rich Plasma During Ascending and Transverse Aortic Arch Surgery ORIGINAL ARTICLES: ADULT CARDIAC
Conclusions The utilization of aPRP was associated with a reduction in allogeneic blood transfusions as well as a decrease in early postoperative morbidity during repairs of the ascending and transverse arch aorta using PHCA.

A lot of drugs and not much oxygen: Is the cocktail responsible for delirium?
We read with great interest the article by Arenson and colleagues that analyzed the risk factors for postoperative delirium, with particular reference to the influence of intensive care unit environment, in a large study population undergoing cardiac surgery. In their multivariate analysis, postoperative delirium was significantly associated with preoperative (eg, age, preadmission use of benzodiazepine), intraoperative (eg, combined coronary artery bypass grafting and valve surgery), and postoperative (eg, stroke or transient ischemic attack, mechanical ventilation>24 hours, blood transfusion, postoperative renal insuffic...

Eptifibatide infusion versus placebo in high risk patients with non-st segment elevation acute coronary syndromes managed with urgent coronary artery bypass graft surgery. a prospective multicenter randomized placebo-controlled clinical trial.
CONCLUSION: Preoperative use of eptifibatide vs. placebo is linked to significantly reduced 12--month MACCE rate in patients with NSTE--ACS requiring urgent CABG, while it simultaneously seems not to confer a greater risk of postoperative bleeding. PMID: 25394877 [PubMed - as supplied by publisher]

Autologous Platelet-Rich Plasma Reduces Transfusions During Ascending Aortic Arch Repair: A Prospective, Randomized, Controlled Trial
Conclusions The use of aPRP reduced allogeneic transfusions during ascending and transverse aortic arch repair with deep hypothermic circulatory arrest. This translated to less acute renal failure, decreased length of stay, and lower transfusion costs. Further studies examining the coagulation factors of aPRP are required.

ISMICS Consensus Conference and Statements of Randomized Controlled Trials of Off-Pump Versus Conventional Coronary Artery Bypass Surgery
Conclusions: OPCAB compared with CCAB may improve outcomes in the short-term (stroke, renal dysfunction, blood transfusion, respiratory failure, atrial fibrillation, wound infection, ventilation time, and length of stay). However, over the longer-term, OPCAB may be associated with reduced graft patency, and increased risk of cardiac re-intervention and death.

Hybrid coronary revascularization versus coronary artery bypass grafting in patients with multivessel coronary artery disease: A meta ‐analysis
ConclusionHCR appears to be safe, and has similar outcomes when compared with conventional CABG. HCR can be a suitable alternative to conventional CABG in select patients with MVCAD. © 2017 Wiley Periodicals, Inc.

Preoperative anemia and outcomes in cardiovascular surgery: systematic review and meta-analysis
ConclusionsPreoperative anemia is associated with adverse outcomes following cardiac surgery. These findings support the addition of preoperative anemia to future risk prediction models, and as a target for risk modification.

Thoracotomy versus sternotomy? The effect of surgical approach on outcomes after left ventricular assist device implantation: A  review of the literature and meta‐analysis
ConclusionsIn the current meta ‐analysis including over 1000 patients undergoing LVAD implantation, a thoracotomy approach was associated with a reduced incidence of RVF (but not RVAD requirement), bleeding, LOS, and mortality. No difference in stroke rates was noted. These findings not only offer additional support as to the f easibility of a thoracotomy approach for LVAD implantation but also suggest a potential superiority over a sternotomy approach.

Right anterior minithoracotomy versus conventional aortic valve replacement: A propensity score matched study
Conclusions: Right anterior minithoracotomy in patients undergoing isolated aortic valve surgery is associated with a lower incidence of postoperative atrial fibrillation and blood transfusion and shorter ventilation time and hospital length of stay. Prospective randomized trials are needed to confirm our data.

Preoperative Hematocrit Is a Powerful Predictor of Adverse Outcomes in Coronary Artery Bypass Graft Surgery: A Report From The Society of Thoracic Surgeons Adult Cardiac Surgery Database.
CONCLUSIONS: Preoperative HCT is a powerful independent predictor of perioperative mortality as well as renal failure and deep sternal wound infection in patients undergoing isolated primary CABG operations. These findings should prompt investigation of strategies to increase preoperative HCT. PMID: 24055236 [PubMed - as supplied by publisher]

110 * a biocompatible perfusion strategy is safe and is associated with excellent clinical outcomes and reduced blood transfusions in a contemporary series of patients undergoing coronary artery bypass grafting: a two-centre study
Conclusions: In a contemporary cohort of patients undergoing CABG, the use of the BPS is safe and effective. It is associated with excellent clinical outcomes and reduced allogeneic blood transfusions.
