Commentary on ‘Plaque Echolucency and the Risk of Ischaemic Stroke in Patients with Asymptomatic Carotid Stenosis Within the First Asymptomatic Carotid Surgery Trial (ACST-1)’
In this issue, the investigators of the first Asymptomatic Carotid Surgery Trial (ACST-1) trial provide a post hoc analysis of a medical therapy group in a randomized trial looking at whether plaque echolucency can predict ipsilateral stroke within a 10 year follow up period.1 Huibers et al. have shown that patients with echolucent plaques had a significantly higher ischemic stroke event rate within 5 years compared with patients with non-echolucent plaques.1 Indeed, carotid plaque echo features have long been suggested to correlate with clinical events in patients with asymptomatic carotid stenosis.

Pioglitazone After Ischemic Stroke or Transient Ischemic Attack
In patients without diabetes but who have insulin resistance along with a recent history of ischemic stroke or TIA, the risk of stroke or myocardial infarction was lower among those patients who received pioglitazone than among those who received placebo.

Endovascular Aortic Arch Repair with Mini-cardiopulmonary Bypass to Prevent Stroke
Debranching thoracic endovascular aortic repair for aortic arch pathology is an important alternative to total arch replacement. However, the problem of intra-operative stroke due to atherosclerotic changes in the aorta remains. We apply our minimally invasive mini-cardiopulmonary bypass system to prevent intra-operative stroke during the endovascular procedure. Once debranching from the right axillary artery to the left common carotid and the left axillary artery is constructed, only the brachiocephalic artery is a pathway to the brain.

Commentary on ‘Risk of Recurrent Stroke in Patients with Symptomatic Mild (20–49% NASCET) Carotid Stenosis’
The authors of this retrospective regional study report that patients with symptomatic mild carotid artery stenosis on current best medical management had a 7.4% cumulative incidence of recurrent ipsilateral stroke in 3-year follow-up.1 This was a significantly higher risk of ipsilateral stroke than that of a comparable set of asymptomatic patients with similar mild carotid stenosis, as well as that of patients with symptomatic moderate or severe stenosis in the 3 years after CEA. If these results are supported by future studies, a prospective randomized trial is needed to determine whether CEA or current best medical therapy is recommended.

Development of a Validated Model to Predict 30-Day Stroke and One-Year Survival After Carotid Endarterectomy for Asymptomatic Stenosis Using the Vascular Quality Initiative
Carotid endarterectomy (CEA) has been shown to be an effective treatment for patients with asymptomatic carotid artery stenosis when perioperative stroke rates are low and patients survive long enough to benefit from the intervention. Our objective was to develop and to validate a simple risk prediction index for 30-day stroke and 1-year mortality to guide optimal selection of patients for CEA.

Impact of Circle of Willis Anatomy in Blunt Cerebrovascular Injury-Related Stroke
Cerebral vascular anatomy, specifically, the circle of Willis (COW), plays an unstudied role in the development of stroke after blunt cerebrovascular injury (BCVI) (carotid and vertebral arteries). Variant anatomy is very common, and certain variants such as persistent fetal circulation (enlarged posterior communicating artery) may improve collateralization between the anterior (carotid) and posterior (vertebral) circulations. Identifying patients at increased stroke risk may allow tailored anticoagulation, the mainstay of therapy.
