Chikungunya: Report from the task force on tropical diseases by the World Federation of Societies of intensive and critical care medicine
Chikungunya is an arbovirus that is transmitted by the Aedes mosquito causing a febrile illness with periodic outbreaks in large parts of the world. In the last decade it has become a public health concern in a host of countries and has affected international tourists. In the vast majority of cases Chikungunya presents as an acute febrile illness, associated with rash, headache, myalgia and debilitating arthralgia or even polyarthritis. A small proportion of patients present atypically with nervous, ocular, renal, myocardial, respiratory and renal system involvement and may require ICU management.

Comparison of ECMO run between H1N1 acute respiratory failure vs. non H1N1 acute respiratory failure
ConclusionsECMO is equally effective in Non H1N1 & H1N1 respiratory failure with much shorter ECMO run in Non H1N1 respiratory failure. Survival with ECMO in tropical infections like Malaria, Dengue & Leptospirosis is more than 60%.

The Lived Experience of ICU Clinicians During the Coronavirus Disease 2019 Outbreak: A Qualitative Study
CONCLUSIONS: Among ICU clinicians, there was a sense of total professional engagement during the surge. Caring for critically ill coronavirus disease 2019 patients was fraught with challenges and generated a strong feeling of responsibility, as clinicians felt they had to compensate for the absence of family members. Rethinking policies about family visits and safeguarding positive relationships among colleagues are two important priorities for future healthcare crises.

Update in sepsis and acute kidney injury 2014.
Abstract Sepsis and acute kidney injury (AKI) represent an important burden in intensive care unit clinical practices. The Journal published important contributions in sepsis for novel therapeutic approaches suggesting that combined molecular targets (e.g., dual inhibition of IL-1β and IL-18, and coadministration of endothelial progenitor cells and stromal cell-derived factor-1α analog) could perform better. The clinical effectiveness of 1,25-dihydroxyvitamin D was reported in a double-blind, randomized, placebo-controlled trial. Although its experimental properties appeared favorable in the pro- and antiinflamm...

Difficult-to-Treat Antibiotic-Resistant Gram-Negative Pathogens in the Intensive Care Unit: Epidemiology, Outcomes, and Treatment
Semin Respir Crit Care Med 2019; 40: 419-434 DOI: 10.1055/s-0039-1696662Antibiotic resistance among gram-negative pathogens is a world-wide problem that poses a constant threat to patients in the intensive care unit and a therapeutic challenge for the intensivist. Furthermore, the substantial economic burden and increased mortality associated with infections due to highly resistant gram-negative pathogens exacerbate these challenges. Understanding the mechanisms, epidemiology, and risk factors for these infections is paramount to the successful control of outbreaks and for guiding therapy which often entails use of antibio...

Targeting PI3K-p110α Suppresses Influenza Viral Infection in Chronic Obstructive Pulmonary Disease.
Conclusions: The increased susceptibility of individuals with COPD to influenza likely results from impaired antiviral responses, which is mediated by increased PI3K-p110α activity. This pathway may be targeted therapeutically in COPD, or in healthy individuals, during seasonal or pandemic outbreaks, to prevent and/or treat influenza. Middle East Respiratory Syndrome.
Abstract Middle East Respiratory Syndrome coronavirus (MERS-CoV) infection has recently spread to Korean Republic (KOR), China, and Thailand. The outbreaks in Korean Republic are characterised by super-spreading events involving several healthcare facilities. The factors leading to the major nosocomial outbreaks in KOR include unfamiliarity among the clinicians with MERS-CoV infection, poor infection control and prevention measures, overcrowding in emergency rooms and medical wards, doctor shopping and invitation of family members and friends to stay in already crowded hospital environment. The role of environment...

NIH Workshop Report: E-cigarette or Vaping Product Use Associated Lung Injury (EVALI): Developing a Research Agenda.
Abstract The National Heart, Lung, and Blood Institute (NHLBI) convened a working group on October 23, 2019 to identify the most relevant and urgent research priorities, and prevailing challenges in e-cigarette or vaping product use associated lung injury (EVALI). Experts across multiple disciplines discussed the complexities of the EVALI outbreak, identified research priorities, and recommended strategies to address most effectively its causal factors, improve diagnosis, treatment, and prevention of this disease. Many research priorities were identified, including the need to create national and international reg...
