What drives the increasing utilisation of hemicraniectomy in acute ischaemic stroke?
Conclusion Utilisation of hemicraniectomy in the USA has increased significantly, in line with compelling results from European clinical trials. Early transfer of patients with malignant infarctions to urban teaching centres could potentially extend the survival benefit to a larger population.

Rates of adverse events in patients with ischemic stroke treated at thrombectomy capable hospitals
Conclusions A decrease in-hospital adverse events and improved discharge outcomes were observed among patients with ischemic stroke admitted to a TCH compared with a non-TCH.

Health disparities and stroke: the influence of insurance status on the prevalence of patient safety indicators and hospital-acquired conditions.
CONCLUSIONS Insurance status is an independent predictor of patient safety events after stroke. Private insurance is associated with lower mortality, shorter lengths of stay, and improved clinical outcomes. PMID: 25658779 [PubMed - as supplied by publisher]

Vascular closure devices in stroke patients receiving tissue plasminogen activator: A retrospective analysis from an academic tertiary medical center and a teaching community hospital stroke database
Stroke is the fourth leading cause of death in the United States [1]. Recanalization, or opening, of the blocked artery in ischemic stroke patients can limit the extent of infarction and significantly improve a patient's outcome [2]. In 1995, NINDS tPA (tissue plasminogen activator) trial suggested that tPA administered intravenously within three hours of stroke onset could improve functional outcomes in patients [3]. Since then, acute stroke therapy has been in constant evolution with the universal goals of improving outcomes and reduction in complications.

Correlation of Upper Extremity Function to Quality of Life of Primary Caregivers of Ambulatory Stroke Survivors Living in the Community
Conclusions: The independently walking stroke patients’ upper extremity and hand function is highly correlated with the quality of life of their primary caregivers. Independence in walking may not be an enhancing factor for the quality of life of the primary caregiver.

E-176 Endovascular stent retriever thrombectomy of medium and small vessel occlusion of the middle cerebral artery with acute ischemic stroke: a case series
ConclusionsOur case series demonstrated that SRMT in SVO with disabling NIHSS is feasible and safe if performed carefully. All patient achieved TICI 3 perfusion and first pass effect was better observed in trap technique and use of long solitaire STD. Good function outcomes were achieved in all cases irrespective of the types of SRD used and passes required to achieve TICI 3 perfusion. Further studies are required.Disclosures Y. Lodi: None.

ConclusionsHospital MT volume significantly influenced the rate of readmissions among AIS patients treated with the EmboTrap® (alone or with another MT device) device. The lack of difference in mortality may also be reflective of limited statistical power to detect differences in study groups. Further research to understand the impact of hospital MT volume on patient outcomes, as well as identify the threshold volume that optimizes positive outcomes, could help inform future stroke care guidelines and contribute to improved treatment protocols in low-volume centers.Disclosures S. Setting up an undergraduate neurology teaching day: 7 steps
The University of Bristol medicine course has one of the shortest neurology placements nationally, just one week. This is despite stroke being the 2nd most common cause of death and neurological illness accounting for 20% of acute medical admissions. Neurology training is set to be reduced even further by GMC regulations limiting clinical placements to the first four years with the final year as a "student assistantship". 199 Mechanical thrombectomy for acute ischaemic stroke - outcomes from Sheffield
Conclusions Outcomes from mechanical thrombectomy at Sheffield teaching hospitals are good, with nearly half of patients having a good functional recovery. However, outcomes in the subgroup of patients aged over 80 were significantly poorer, with high complication and mortality rates; this warrants further study.

Care of the TIA patient
I read with interest Al-Khaled et al. study on the prognostic impact of the stroke unit (SU) concept in patients presenting after a transient ischemic attack (TIA). They found no significant impact of SU care as compared to conventional care (CU) in TIA patients. Their study raises some interesting observations. The conventional teaching is that patients with TIA warrant emergent evaluation to identify and modify their stroke risk factors. Evaluation of time to aneurysm treatment following subarachnoid hemorrhage: comparison of patients treated with clipping versus coiling
Conclusions Older patients with SAH and those treated at non-teaching hospitals were more likely to have delays to aneurysm clipping procedures. These associations were unique to open surgery as age and hospital teaching status did not affect time to coiling procedures.
