How John Fetterman Came Out of the Darkness
When he looks back on the past year—a year in which he nearly died, became a U.S. Senator, and nearly died again—it is the debate that John Fetterman identifies as the breaking point. “The debate lit the mitch,” he says, then shakes his head in frustration and tries again. The right word is there in his brain, but he struggles to get it out. “Excuse me, that should be lit the mitch—” He stops and tries again. “Lit the match,” he says finally. Oct. 25, 2022: the date is lodged in his mind. “I knew I had to do it,” he tells me. “I knew that the vote...

Genes, Vol. 14, Pages 1448: Isoflurane Anesthesia & rsquo;s Impact on Gene Expression Patterns of Rat Brains in an Ischemic Stroke Model
Conclusions: We were able to determine the effects of the duration of anesthesia using isoflurane on the transcriptomes in the brains of rats at 24 h after tMCAO. Thus, specific genome responses may be useful in developing potential approaches to reduce damaged areas after cerebral ischemia and neuroprotection.

The association between intraoperative fluid management and perioperative allogenic blood transfusion during adolescent idiopathic scoliosis surgery
CONCLUSION: These results indicate a statistical association between the increase in crystalloid intake and the risk of allogenic blood transfusion during surgical correction of adolescent idiopathic scoliosis. Controlled studies are needed to address the causative relation between intraoperative fluid intake and the risk of allogenic transfusion.PMID:37386846 | DOI:10.1111/pan.14722

Association of cervical atherosclerosis on Doppler ultrasonography and postoperative delirium in patients undergoing spinal surgery: a single-center, retrospective, observational study
CONCLUSIONS: There was a significant association between POD and the prevalence of cervical atherosclerosis using the univariate logistic regression analysis. Furthermore, multivariate logistic regression analyses showed that older age and antiplatelet agent use were independently associated with POD.PMID:37326838 | DOI:10.1007/s00586-023-07823-0

The Effect of Intraoperative Hypotension on Postoperative Renal Function
AbstractPurpose of ReviewThis review summarizes the most recent literature on the association between intraoperative hypotension (IOH) and the occurrence of postoperative acute kidney injury (AKI). It provides recommendations for the management of intraoperative blood pressure to reduce the incidence of postoperative AKI. Fluid management strategies, administration of vasopressor medications, and other methods for reducing the incidence of AKI are also briefly discussed.Recent FindingsRecent retrospective studies have demonstrated a solid association of IOH with postoperative AKI. IOH is associated not only with AKI but al...

General anesthesia or conscious sedation for thrombectomy in stroke patients: an updated systematic review and meta-analysis
CONCLUSIONS: In this updated systematic review and meta-analysis, the choice of anesthetic strategy for endovascular treatment of stroke patients did not significantly impact functional outcome as measured with the mRS at three months. Patients managed with general anesthesia may have more frequent successful reperfusion.TRIAL REGISTRATION: PROSPERO (CRD42022319368); registered 19 April 2022.PMID:37268801 | DOI:10.1007/s12630-023-02481-8
