Off-pump coronary artery bypass grafting in left main stem stenosis: outcomes, concerns and controversies.
Authors: Athanasopoulos LV, Athanasiou T Abstract Left main stem (LMS) disease is known to be a poor prognostic factor in terms of morbidity or mortality. Traditionally, it has been treated with constitution of bypass to provide required haemodynamic stability. We searched the literature for evidence on off-pump (OFP) surgery for treating this high-risk group of patients focusing in our review on postoperative outcomes, concerns and controversies. The majority of the studies identified showed favourable or equal outcomes of OFP when compared to conventional approach. All of the studies, apart from two, which showed...

Outcome of extracorporeal membrane oxygenation support for complex high-risk elective percutaneous coronary interventions: A single-center experience
Conclusions Elective high-risk PCI supported by ECMO is a viable alternative for patients who are at very high risk for CABG.

Current outcomes of off-pump versus on-pump coronary artery bypass grafting: evidence from randomized controlled trials.
Authors: Fudulu D, Benedetto U, Pecchinenda GG, Chivasso P, Bruno VD, Rapetto F, Bryan A, Angelini GD Abstract Coronary artery bypass grafting remains the standard treatment for patients with extensive coronary artery disease. Coronary surgery without use of cardiopulmonary bypass avoids the deleterious systemic inflammatory effects of the extracorporeal circuit. However there is an ongoing debate surrounding the clinical outcomes after on-pump versus off-pump coronary artery bypass (ONCAB versus OPCAB) surgery. The current review is based on evidence from randomized controlled trials (RCTs) and meta-analyses of ra...

Impact of sleep disordered breathing on short-term post-operative outcome after elective coronary artery bypass graft surgery: a prospective observational study
Sleep disordered breathing (SDB) is common in patients with coronary disease, but its impact on post-operative recovery after coronary artery bypass graft surgery (CABG) is unclear. We therefore determined the effects of SDB on post-operative outcome after elective CABG. In this prospective two-centre study, 219 patients due to receive elective CABG underwent cardiorespiratory polygraphy for SDB prior to surgery and were monitored for post-operative complications. The primary end-point was a composite of 30-day mortality or major post-operative complications (cardiac, respiratory, surgical, infectious, acute renal failure ...

On-pump beating heart versus conventional coronary artery bypass grafting: comparative study on early and long-term clinical outcomes.
Conclusions: The OPB strategy may be as safe and effective as the conventional strategy during CABG among patients with similar risk profiles. A prospective randomized trial is warranted to better ascertain the beneficial impact of OPB-CABG as both a viable and a durable alternative strategy to C-CABG. PMID: 29997927 [PubMed]

Triglyceride glucose index for predicting cardiovascular outcomes in patients with coronary artery disease.
Conclusions: The present study, firstly, showed that TyG index was positively associated with future CVEs, suggesting that TyG may be a useful marker for predicting clinical outcomes in patients with CAD. PMID: 30622785 [PubMed]

Secondary prevention advices after cardiovascular index event: From drug prescription to risk factors control in real world practice.
Authors: Faggiano P, Fattirolli F, Frisinghelli A, Piccioli L, Dasseni N, Silverii MV, Albricci L, D'Ambrosio G, Garrì R, Esposito L, Giallauria F Abstract The present study aims at evaluating the achievement of blood pressure, lipid and blood glucose targets, healthy lifestyle changes and appropriate drug prescription/adherence in patients attending secondary prevention/CR ambulatory visit after index cardiovascular event in a time period ranging 1 to 5 year. At ambulatory visit, a predetermined set of data collection was used, including demographic data, cardiovascular risk factors and lifestyle habits, type and...

Pulmonary complications after elective coronary artery bypass grafting surgery in patients with and without sleep-disordered breathing - a prospective observational study
Conclusion: Amongst established risk factors for PPCs, central sleep apnoea was independently associated with MPCs.

Early surgical myocardial revascularization in non-ST-segment elevation acute coronary syndrome.
Conclusions: Early revascularized patients showed a higher level of illness. However, results of early CABG were comparable to those following delayed revascularization. Moreover, EuroSCORE II was determined as independent risk factors for mortality. PMID: 31903232 [PubMed]

Association of early elevated cardiac troponin I concentration and longitudinal change after off-pump coronary artery bypass grafting and adverse events: a prospective cohort study.
Conclusions: Longitudinal change in cTnI levels within 24 hours and early cTnI concentrations, even less than 1 hour after CABG, are associated with adverse events. These data will be useful in identifying patients at an increased risk of complications. PMID: 33282356 [PubMed]

Beating-heart on-pump coronary artery bypass grafting < em > vs. < /em > off-pump coronary artery bypass grafting: a systematic review and meta-analysis
CONCLUSIONS: BH-ONCAB is a safe and comparable alternative to OPCAB in terms of early mortality and late survival. BH-ONCAB may confer particular advantages in preventing incomplete revascularization and allowing more distal anastomoses compared to OPCAB. However, BH-ONCAB was associated with more postoperative complications due to the use of CPB. Future work should focus on larger matched studies and multicenter randomized controlled trials to optimize our surgical revascularization strategies.PMID:34422347 | PMC:PMC8339796 | DOI:10.21037/jtd-21-268

Ninety-seven cases of experiences with the left thoracotomy approach for off-pump conventional revascularization: a retrospective cohort study
CONCLUSIONS: Left thoracotomy for conventional revascularization is a safe procedure for appropriately selected patients, with few early complications and good early and mid-term results. More cases are ongoing and long-term results are in observation.PMID:36389332 | PMC:PMC9641355 | DOI:10.21037/jtd-22-1162
