CHA2DS2-VASc Score for Identifying Truly Low-Risk Atrial Fibrillation for Stroke. A Korean Nationwide Cohort Study
Prevention of stroke in patients with atrial fibrillation (AF) is the main priority in clinical management. However, the use of oral anticoagulation is not without risk and the net clinical benefit should be considered before starting patients on anticoagulation. Several risk stratification tools have historically been used to help identify patients at high risk for ischemic stroke, however the focus is shifting to identifying truly low-risk patients in whom anticoagulation can be avoided.

Observational Multicenter Study of a Direct-to-CT Protocol for EMS-transported Patients with Suspect Stroke
In patients with suspected stroke, computed tomography (CT) scan is a critical rate-limiting step in determining whether or not to administer thrombolysis. In recent years, some facilities have adopted the practice of emergency medical service (EMS) providers transporting stable patients with suspected stroke directly to the CT scanner, bypassing traditional triage and emergency department assessment. There is a paucity of research and data evaluating the direct-to-CT practice and associated outcomes.

Validation of SUSPEKT Score in Predicting One-month Mortality of Patients with Hemorrhagic Stroke; a Diagnostic Accuracy Study.
Conclusion: Total accuracy of SUSPEKT score in predicting one-month mortality of non- traumatic ICH patients is in good range and it has 82% sensitivity and 92% NPV in this regard. It seems that we need further studies before applying the score in routine practice. PMID: 31875210 [PubMed]

Limitations of Prehospital Stroke Scales for Large-Vessel Occlusion Detection
We read with great interest Guillory et  al.’s study of prehospital use of the Field Assessment Stroke Triage for Emergency Destination (FAST-ED) scale for detection of large-vessel occlusions (LVO) (1). Although we acknowledge that the study is limited by an unblinded protocol, we believe that these preliminary findings hold importanc e. This is due to the ongoing need for a reliable and well-validated protocol for early detection and triage of LVO stroke cases to specialist centers for endovascular thrombectomy.
