Survey of primary care providers ’ knowledge of screening for, diagnosing and managing prediabetes
ConclusionsAddressing gaps in knowledge and the underutilization of behavioral weight loss programs in prediabetes are two essential areas where PCPs could take a lead in curbing the diabetes epidemic.

Glycemic control of type 2 diabetes mellitus across stages of renal impairment: information for primary care providers.
Authors: Tong L, Adler S Abstract Chronic kidney disease (CKD) is a frequent complication of type 2 diabetes mellitus (T2DM) and elevates individuals' risk for cardiovascular disease, the leading cause of morbidity and mortality in T2DM. Achieving and maintaining tight glycemic control is key to preventing development or progression of CKD; however, improving glycemic control may be limited by effects of renal impairment on the efficacy and safety of T2DM treatments, necessitating dosing adjustments and careful evaluation of contraindications. Understanding the treatment considerations specific to each class of T2D...

Primary care management of type 2 diabetes: a comparison of the efficacy and safety of glucagon-like peptide-1 receptor agonists and dipeptidyl peptidase-4 inhibitors
Postgrad Med. 2021 Aug 20. doi: 10.1080/00325481.2021.1971461. Online ahead of print.ABSTRACTGlucagon-like peptide-1 receptor agonists (GLP-1RAs) and dipeptidyl peptidase-4 inhibitors (DPP4is) exert their effects via the incretin system, which augments glucose-dependent insulin secretion in response to nutrient intake (the 'incretin effect'). Both classes are well-established pharmacologic options for the management of glycemic control in individuals with type 2 diabetes (T2D) after failure of first-line metformin; however, they have inherent differences in their mechanisms of action that are reflected in their clinical sa...

Trends in the Degree of Control and Treatment of Cardiovascular Risk Factors in People With Type 2 Diabetes in a Primary Care Setting in Catalonia During 2007-2018
CONCLUSIONS: During the 2007-2018 period, relevant improvements in blood pressure and lipid control occurred, especially in people with cardiovascular disease. Despite the increase in the use of antidiabetic and cardiovascular drugs, the proportion of patients in which the three objectives were simultaneously achieved is still insufficient and plateaued after 2013. The use of antidiabetic drugs with demonstrated cardio renal benefits (SGLT-2 and GLP-1ra) increased over the years, but their use remained quite low.PMID:35082758 | PMC:PMC8784522 | DOI:10.3389/fendo.2021.810757

Clinical presentations and prognosis of metformin-associated lactic acidosis patients in the intensive care unit: A 20-year survey
Conclusions: Despite MALA being a rare event, it is necessary to evaluate its clinical characteristics, especially the associated PLL and mortality. In the current study, higher levels of APACHE II score and PLL show a greater likelihood of mortality in MALA patients. Metformin-associated lactic acidosis (MALA) is a rare but serious adverse event. It is associated with a high mortality rate and is diagnosed by the peak lactate level (PLL). This study examines the relationship between the clinical presentations and PLL in patients under metformin therapy admitted to the intensive care unit (ICU) to better diagnose MA...

Nonalcoholic steatohepatitis diagnosis and treatment from the perspective of patients and primary care physicians: a cross-sectional survey
CONCLUSIONS: PCPs may benefit from greater awareness of NASH and guidelines for its diagnosis and treatment. Given the absence of pharmacological treatments approved for NASH, PCPs can offer support in obesity management, comorbidity management, and risk stratification for liver disease progression.PMID:37171239 | DOI:10.1080/07853890.2023.2211349

Dashboards to reduce inappropriate prescribing of metformin and aspirin: A quality assurance programme in a primary care sentinel network
CONCLUSIONS: It was feasible to use a dashboard to flag inappropriate prescribing. Whilst underpowered to report a change in metformin, we demonstrated a reduction in inappropriate aspirin prescribing.PMID:34147402 | DOI:10.1016/j.pcd.2021.06.003

Metformin-based single pill drug combinations for type 2 diabetes in primary care England: A time trend analysis
CONCLUSIONS: Overall, an increase in prescription items can be seen for metformin-based single pill drug combinations along with an increase in their costs in primary care in England between 2015 and 2020. There was a declining trend for the number of ADRs reported per million prescription items dispensed for metformin-containing single pill combinations, even though their prescription rate increased.PMID:35115253 | DOI:10.1016/j.pcd.2022.01.008
