Stroke: New study vindicates use of low-dose alteplase in East Asia
Nature Reviews Neurology 11, 486 (2015). doi:10.1038/nrneurol.2015.152 Alteplase at 0.6 mg/kg body weight produces comparable outcomes to the standard 0.9 mg/kg dose in Korean patients with ischaemic stroke, a recent study reveals. Use of low-dose alteplase is common in East Asia, owing to concerns over the risk of cerebral haemorrhage, but few

Statistical analysis plan for evaluating low‐ vs. standard‐dose alteplase in the ENhanced Control of Hypertension and Thrombolysis strokE stuDy (ENCHANTED)
ConclusionsWe have developed a predetermined statistical analysis plan for the ENhanced Control of Hypertension And Thrombolysis strokE stuDy alteplase dose arm which is to be followed to avoid analysis bias arising from prior knowledge of the study findings.

Intravenous Recombinant Tissue Plasminogen Activator Does Not Impact Mortality in Acute Ischemic Stroke at Any Time Point up to 6 Months: A Systematic Review and Meta-Analysis of Randomized Controlled Clinical Trials
Conclusion Intravenous alteplase did not impact mortality in patients with acute ischemic stroke at any of the measured time points up to 6 months (i.e., there was no increase in the risk of death with alteplase). Therefore, intravenous alteplase should be given to all eligible patients with acute ischemic stroke to improve long-term neurologic outcomes. The effects of alteplase on early survival are more complex than previously understood.

Uric Acid Therapy Improves Clinical Outcome in Women With Acute Ischemic Stroke Clinical Sciences
Conclusions— In women with acute ischemic stroke treated with alteplase, the administration of UA reduced infarct growth in selected patients and was better than placebo to reach excellent outcome. Clinical Trial Registration— URL: https://clinicaltrials.gov. Unique identifier: NCT00860366.

Effect of Intravenous Recombinant Tissue-Type Plasminogen Activator in Patients With Mild Stroke in the Third International Stroke Trial-3: Post Hoc Analysis Brief Reports
Conclusions— This post hoc analysis in a highly selected sample of IST-3 supports the rationale of A Study of the Efficacy and Safety of Activase (Alteplase) in Patients With Mild Stroke (PRISMS) trial—a randomized, phase IIIb study to evaluate IV r-tPA in mild ischemic stroke.

A pragmatic approach to sonothrombolysis in acute ischaemic stroke: the Norwegian randomised controlled sonothrombolysis in acute stroke study (NOR-SASS)
DiscussionNOR-SASS is the first randomised controlled trial designed to test the superiority of contrast enhanced ultrasound treatment given ≤4½ hours after stroke onset in an unselected acute ischaemic stroke population eligible or not eligible for intravenous thrombolysis, with or without a defined arterial occlusion on CTA. If a positive effect and safety can be proven, contrast enhanced ultrasound treatment will be an option for all acute ischaemic stroke patients. To determine the impact of serum albumin levels on admission with the outcomes in patients receiving IV-t-PA (alteplase) for treatment of acute ischemic stroke. (P5.134)
Conclusion: Among ischemic stroke patients treated with thrombolysis admission hypoalbuminemia was not a predictor of poor outcome. This will need to be validated in larger studies.Disclosure: Dr. Limaye has nothing to disclose. Dr. Lahoti has nothing to disclose. Dr. Hinduja has nothing to disclose.

Arterial Reocclusion And Distal Embolization During Endovascular Treatment Using New Generation Stent Retrievers In Acute Ischemic Stroke Patients (P4.299)
CONCLUSIONS: Arterial reocclusion and distal embolization occur in 16[percnt]-18[percnt] of acute ischemic stroke patients undergoing mechanical thrombectomy with new stent retrievers. The rates are no higher than those observed in cohorts treated without stent retrievers.Disclosure: Dr. Herial has nothing to disclose. Dr. Qureshi has nothing to disclose. Burden of stroke in Italy: An economic model highlights savings arising from reduced disability following thrombolysis
ConclusionOur study reveals that performing thrombolytic therapy in eligible patients improves economic outcomes compared with patients receiving standard care. This model is useful for decision makers, both within and outside of the Italian national context, as a tool to assess the cost‐effectiveness of thrombolysis in both short‐ and long‐term period.
