The Sky's the Limit: Expanding Nursing's Contribution to Acute Stroke Science
Am J Crit Care. 2022 Jul 1;31(4):266-274. doi: 10.4037/ajcc2022109.ABSTRACTStroke is the number one cause of preventable disability in adults in the United States. Significant advances have occurred in medications and technology supporting rapid stroke diagnosis and treatment during the past 30 years, along with blurring of the lines of what traditionally constituted nursing or medical research. Ischemic stroke is a disease of vascular insufficiency that mirrors myocardial infarction more than any other neurologic diagnosis. My primary program of research is focused on exploration of methods to improve intracranial blood f...

Novel Use of a Nurse-Led Telemedicine Team in Acute Stroke: A Retrospective Review of the Impact on a Regional Health Care System
ConclusionIn an acute stroke telemedicine system, implementation of a local nurse-led “stroke responder” system resulted in significantly decreased acute stroke metrics for a community hospital within a regional hospital system.

Providing Stroke and Hypertension Education in Amharic for Ethiopian Persons Living in Dallas, Texas, United States.
CONCLUSION: This study did not show a statistically significant increase in knowledge about stroke when presented learning materials in subjects' native language compared to in English. The use of video to present stroke and stroke-risk educational content can be used in future research and global health initiatives to increase stroke knowledge in the Amharic-speaking community. A Nursing Approach to Improving Critical Care Compliance With Vital Signs and Neurological Assessments in Post-IV-Alteplase Stroke Patients
Ischemic stroke represents 87% of all strokes. As global initiatives move forward with stroke care, health care providers and institutions will be called on to deliver the most current evidence-based care. The American Heart Association/American Stroke Association (AHA/ASA) estimates that 795 000 strokes occur each year; 610 000 are new strokes, and 185 000 are recurrent strokes. Eighty-seven percent are ischemic strokes; the overall mortality rate from stroke was 273 000, which makes stroke the fifth leading cause of death and the leading cause of disability in the United States. Stroke costs the United States an estimate...

Sex Differences in Prehospital Delay in Patients With Acute Stroke: A Systematic Review.
CONCLUSIONS: Most study authors found no differences in prehospital delay between women and men; however, women delayed longer in some Asian-Pacific and American studies. Findings of sex differences were inconclusive. PMID: 32649376 [PubMed - as supplied by publisher]

Novel Use of a Nurse-Led Telemedicine Team in Acute Stroke: A Retrospective Review of the Impact on a Regional Health Care System
Despite the increasing incidence of acute ischemic stroke in the United States, many health care facilities remain unprepared to manage patients with acute stroke, including the administration of intravenous alteplase (recombinant tissue plasminogen activator [rTPA]). This has led to an opportunity for telemedicine systems to facilitate these evaluations and acute medical stroke treatment decisions. However, even telemedicine systems can fail to provide timely evaluation and management of the patient with acute stroke.

Indications and Evidence for Dual Antiplatelet Therapy After Acute Ischemic Stroke
The antiplatelet landscape for the secondary prevention of ischemic stroke has changed significantly over the past decade. Poststroke dual antiplatelet regimens are becoming increasingly routine as supported by recent literature and guideline recommendations. Dual antiplatelet therapy after stroke generally consists of aspirin and clopidogrel and is considered in the short term after stroke in select populations including those with mild stroke or transient ischemic attack and in patients with severe intracranial atherosclerosis. When initiating dual antiplatelet therapy, factors that may increase a patient's risk of bleed...

Large Vessel Occlusion in the Acute Stroke Patient
Acute ischemic stroke is a major cause of death and disability in the United States. Historically, acute stroke patients were treated with intravenous (IV) thrombolysis. Patients with large vessel occlusions (LVOs) should be offered mechanical thrombectomy, with or without IV thrombolysis, in an extended window up to 24  hours of last known well. Both treatment options are the standard of care for a patient with an LVO. It is critical that the intensive care unit nurse understand new treatment indications for LVO strokes, and the priorities of nursing care with medical and endovascular intervention.

Preface
A stroke can happen to anyone at any time. Every year, nearly 800,000 Americans suffer a stroke. Despite improvements in stroke treatment in the United States, stroke remains the second leading cause of death worldwide. Stroke is also the third-leading cause of death and disability combined. Worldwide, the annual rate of stroke is 15 million. Of these, 3 million die and another 3 million are left permanently disabled. Stroke occurs every 40 seconds. Rapid and effective evaluation, identification, diagnosis, and treatment of stroke are vital in decreasing stroke morbidity and mortality.

Review of Article: Rivaroxaban with or without Aspirin in Stable Cardiovascular Disease. Eikelboom JW, Connolly SJ, Bosch J, et  al. for the COMPASS Investigators
Cardiovascular disease accounts for nearly 801,000 deaths in  the United States.1 Patients with vascular disease have a 20%-60% increased risk of a myocardial infarction (MI), a 40% increased risk of a stroke, and a 2- to 6-fold increase of death due to cardiovascular disease.2,3 The primary goal in antiplatelet therapy is to reduce the risk of MI, stroke, o r vascular death in patients with atherosclerosis and has been the cornerstone in medical management of the patient with cardiovascular disease.
