Effect of age on arterial recanalization and clinical outcome in thrombolyzed acute ischemic stroke in CLOTBUST cohort
Conclusion: Elderly AIS patients achieve relatively poor functional outcome after IV-tPA despite similar rates of arterial recanalization. However, there is no increase in the hemorrhagic risk. Perhaps, decision for IV thrombolysis in elderly patients should be made cautiously.

Endovascular Treatment for Acute Tandem Occlusion Stroke: Results from Case Series of 17 Patients
Conclusion: Our study determined a promising outcome with reasonable good recanalization and clinical recovery for endovascular intervention in tandem ischemic. In the subgroup of treatment, “extracranial stent first” had more complex disease with completed ICA occlusion which required longer procedure time may lead to worse outcome.

Mechanical thrombectomy: Answering unanswered
Rajsrinivas Parthasarathy, Vipul GuptaAnnals of Indian Academy of Neurology 2020 23(1):13-19 The stroke physician community witnessed a major “breakthrough” in acute stroke therapeutics when the results of the first of the many positive trials, “MR CLEAN,” were published showing a significant absolute benefit in favor of mechanical thrombectomy in patients with large vessel occlusion (LVO). Thereafter, the investigators of ESCAPE, SWIFT PRIME, REVASCAT, THRACE, and PISTE concluded the same. Based on the initial studies, the American Stroke Association amended the 2013 gu...

Role of 24-hr blood pressure variability as a target therapeutic risk factor for poor functional outcome of acute ischemic stroke
Conclusions: Reduction in BP variability might have a beneficial impact on the outcome of patients with acute ischemic stroke. There is further scope to explore optimum therapeutic strategies to minimize BPV in the management of acute ischemic stroke.

Perioperative Stroke in Carotid Artery Stenting as a Surrogate Marker and Predictor for 30-day Postprocedural Mortality & #8211; A Pooled Analysis of 156,000 Patients with Carotid Artery Disease
Conclusions: Perioperative stroke drastically increases the risk of 30-day mortality. The occurrence of perioperative stroke exhibited high specificity but modest sensitivity in predicting 30-day mortality following CAS. This highlights the importance of neurophysiologic monitoring to detect intraoperative cerebral ischemia and perform timely interventions.

What Happens to Swallowing Muscles after Stroke ? : A Prospective Randomized Controlled Electrophysiological Study
Conclusion: Although swallowing is examined as different phases, the process is complicated and should be evaluated totally. In post-stroke dysphagia, oral phase of swallowing process is as important as phayngeal phase and perioral, mastication, and tongue muscles are influenced even in an early period as a month.

A Comparison of Intracerebral Transplantation of RMNE6 Cells and MSCs on Ischemic Stroke Models
Conclusion: MSCs are still the preferred cells for cell replacement in stroke therapy, while RMNE6 cells need to be modified.

Incidence and Symptomatology of Vascular Crossed Aphasia in Bengali
Conclusions: The relatively high incidence of CA in our study suggests that bi-hemispheric language representation may be more prevalent in Bengali speakers than in speakers of other languages. The absence of crossed Wernicke aphasia in our study participants may represent a left-hemispheric advantage for receptive language abilities in Bengali speakers. Further studies are required to clarify whether idiosyncrasies in the Bengali language may be responsible for the differential brain representation of language seen in our study participants.

Vascular Endothelial Growth Factor as Predictive Biomarker for Stroke Severity and Outcome; An Evaluation of a New Clinical Module in Acute Ischemic Stroke
Conclusion: This study demonstrates that VEGF is highly upregulated in AIS with severe disability as compared to healthy controls. This biomarker is a strong predictor of severity and functionality when combined with clinical variables three months post the ishemic event.

Critical Issues and Recent Advances in Anticoagulant Therapy: A Review
Angelika Batta, Bhupinder S Kalra, Raj KhirasariaNeurology India 2019 67(5):1200-1212 As the population is aging, clinicians are coming across more patients with atrial fibrillation and venous thromboembolism requiring anticoagulation to prevent stroke and systemic embolisms. Due to a high prevalence and unfavorable consequences, managing thromboembolic diseases have become areas of clinical concern. Traditional anticoagulants like heparin, low molecular weight heparin and warfarin have been used for the prevention and treatment of venous and arterial thromboses. But, issues of bleeding, parenteral route of administration...

Epidemiologic features, risk factors, and outcomes of respiratory infection in patients with acute stroke
Conclusions: Pneumonia seems to be a major complication in stroke patients. A better understanding of its risk factors is important for prevention and early recognition. Further studies are needed to clarify the optimal preventive treatment for SAP patients.

Post-stroke Visual Gait Measure for Developing Countries: A Reliability and Validity Study
Conclusion: Video-based RVGA is a reliable and valid tool to assess gait-related impairment in post-stroke hemiparesis. This cost-effective measure may be incorporated in the clinical and research practice to discern and quantify complex phenomenon of the gait deviation. RVGA may be considered as a useful tool, especially in developing countries where expensive gait analyzer is usually not available.

Antiplatelets versus Anticoagulants in the Treatment of Extracranial Carotid and Vertebral Artery Dissection
Conclusions: The risk of recurrent ischemic events in carotid and vertebral artery dissection is low and is irrespective of the type of antithrombotic treatment. Stroke as the presenting event and severity of stroke at onset were significant predictors of a poor three-month outcome.

Etiologies of simultaneous cerebral infarcts in multiple arterial territories: A simple literature-based pooled analysis
Conclusion: Acute multiple infarcts in multiple cerebral circulations occur among 13% of stroke patients. Although cardioembolism is the most common cause, there are several other etiologies that require a different workup and secondary prevention strategy.
