Influence of geography, stroke timing, and weather conditions on transport and workflow times: Results from a longitudinal 5-year Canadian provincial registry
CONCLUSION: Geographic factors and stroke timing were associated with EVT workflow times. However, no association with clinical outcomes was seen, suggesting that EVT patients living remote areas of Saskatchewan have similar benefit from EVT compared to urban areas. Every effort should be made to offer timely EVT to patients from remote areas.PMID:37608547 | DOI:10.1177/15910199231196614

A Stitch in Time Saves 1.9 Million: How Advanced Imaging Solutions in Neuroradiology can Improve Outcomes for Stroke Patients
Stroke is the leading cause of disability in Canada and the third leading cause of death. Every year there are over 50,000 new strokes, with nearly 14,000 of cases resulting in death. For every minute delay in treating a stroke, the patient loses 1.9 million brain cells, and for each hour in which treatment does not occur, the brain loses as many neurons as it does in almost 3.6 years of normal aging. Thus, it is vital that the patient receives revascularization therapy as soon as possible. Despite this knowledge, there is still an inefficient use of time between stroke onset to revascularization.

A systematic review of cost-effectiveness analyses on endovascular thrombectomy in ischemic stroke patients
ConclusionsOur study reveals significant heterogeneity in previously published thrombectomy CEAs, highlighting need for better standardization in future CEAs.Key Points• All included studies concluded thrombectomy to be cost-effective, from both long- and short-term perspectives.• Only 5 out of 22 studies undertook a societal perspective, and only 1 accounted for indirect costs.• The range of value for mRS 0–2 was 0.71–0.85 quality-adjusted life year (QALY) and 0.21–0.40 QALY for mRS 3–5.

Antithrombotic Use Predicts Recanalization of Embolized Pulmonary  Arteriovenous Malformations in Hereditary Hemorrhagic Telangiectasia
Pulmonary arteriovenous malformations (PAVMs) are most commonly seen in association with hereditary hemorrhagic telangiectasia (HHT), also known as Osler-Weber-Rendu syndrome [1]. Standard treatment is coil embolization, as PAVMs can cause stroke and cerebral abscesses [2] due to a paradoxical shunt, and pulmonary complications such as spontaneous hemothorax and hemoptysis. PAVM recanalization is an important finding that may lead to paradoxical embolism. Previous studies [3,4] have demonstrated increased feeding artery diameter, low number of coils, use of oversized coils, and proximal coil placement within the feeding ar...

Management strategies of unanticipated intracranial stenosis during mechanical thrombectomy for acute stroke: A survey of academic neurointerventionalists
CONCLUSION: The approach to underlying ICAD in LVO varies widely; however, the majority of neurointerventionalists prefer medical therapy with DAPT as a first-line treatment approach. The current survey highlights the need for studies that better define the optimal timing and modality of treatment, along with an evidence-based framework for balancing the risks associated with these treatment approaches.PMID:35758313 | DOI:10.1177/15910199221110971
