Flow down gradients: the problem of pressure in this physiology core concept
Adv Physiol Educ. 2023 May 4. doi: 10.1152/advan.00107.2022. Online ahead of print.ABSTRACTThe core concepts of physiology, as first published in this journal in 2011, not only provide a noteworthy teaching approach but also encourage reflection on the fundamentals of physiology. Unfortunately, a fundamental flaw has crept into the core concept of flow down gradients. Fluids do not generally flow from high to low pressure, as claimed, but only because of a specific pressure difference, that is, the perfusion pressure. This is related to a problem that is widespread in physiology, from which even the core concepts are not f...

The long-term prognostic value of left ventricular dyssynchrony assessed by phase analysis of 99mTc-MIBI gated SPECT in patients with left ventricular aneurysm:A comparative study of medical and surgical treatment
Conclusions: Patients with LV aneurysm and LVMD had a worse prognosis, coronary revascularization is associated with improved long-term survival in compared with medical therapy. In contrast, patients without LVMD had a good long-term survival, revascularization improved cardiac-event survival in comparison with medical therapy.

Usefulness of thrombolysis in cardiac arrest secondary to suspected or confirmed pulmonary embolism
This study describes use patterns of systemic thrombolysis in cardiac arrest and corresponding patient outcomes. A multicenter retrospective chart review was conducted to evaluate adult patients who received rescue thrombolysis during cardiac arrest for suspected or confirmed PE. A total of 27 patients were included. PE was confirmed in 4 patients (15%). Pulseless electrical activity was the initial rhythm in 21 patients (78%), with a median cardiac arrest duration of 23 minutes in patients with return of spontaneous circulation (ROSC) vs 42.5 minutes in patients without ROSC. Among the 11 patients (41%) with ROSC, two (7%...
