Does the site of magnetic resonance imaging abnormalities match the site of recent-onset inflammatory back pain? The DESIR cohort
Conclusions The site of pain (thoracic spine, lumbar spine or buttock(s)) is associated with MRI inflammation at the same site in patients with recent IBP.

Alliaceous Migraines.
CONCLUSION.-: This is the first report of migraines with aura triggered by more than one alliaceous compound in the same individual. Possible mechanisms include odor induced, emotional change, vasomotor instability, trigeminal-induced neurogenic inflammation, and allergic response. In alliaceous and odor-induced migraines, a trial of counter stimulation and nose plugs is warranted. PMID: 23551212 [PubMed - as supplied by publisher]

The ideal puncture approach for PCNL: Fluoroscopy, ultrasound or endoscopy?
Bannakij LojanapiwatIndian Journal of Urology 2013 29(3):208-213Percutaneous renal access is a common procedure in urologic practice. The main indications are drainage of an obstructed and hydronephrotic kidney and antegrade renal access prior to percutaneous renal surgeries such as percutaneous nephrolithotomy (PCNL) and percutaneous endopyelotomy (EP). The contraindications for this technique are patients with history of allergy to topical or local anesthesia and patients with coagulopathy. The creation of a percutaneous tract into the renal collecting system is one of the important steps for percutaneous renal access. T...

An unusual case of progressive cerebellar ataxia
Conclusion Oculomotor apraxia, although included in the acronym, is not obligatory for diagnosis of AOA2. A range of movement disorders including tremor, chorea and dystonia may be present in approximately 14% of the patients.1 AFP level may occasionally be in the upper range of normal and may increase with time. AFP is an important biomarker for recessive ataxias and should be included in the investigation of recessive cerebellar ataxias or adolescent movement disorders of unknown aetiology.

Weight loss, encephalopathy, urinary difficulties and numb feet in a nigerian man
We described the first case of the TTR Leu12Pro mutation in 1999. To date it has been only described in a total of 3 patients, all European. Here we describe the first report of the TTR Leu12Pro variant in an African patient.

Faciobrachial dystonic seizures: two cases alike, but not the same
Conclusion Our cases emphasise the different clinical features, different imaging features and EEG's seen with this condition and we hope to raise awareness of the importance of early clinical recognition, investigation and appropriate immunosuppresion.

Probing the potential for functional recovery in patients with neuromyelitis optica using visuomotor practice
Conclusion Refuting our initial hypothesis, we found that visuomotor improvements in patients were comparable to those of healthy volunteers, suggesting that plasticity is preserved in NMO. Damage to functional "bottlenecks" did not appear to be a factor in preventing visuomotor improvement with practice, suggesting that the limits of adaptive plasticity in NMO may result from a complex interaction between distribution, location and type of damage. The effect of cognitive dysfunction on the ability to improve performance in the more cognitively demanding task condition emphasises the importance of tailoring recovery–...

Most neurologists in scotland do not use the mcdonald 2010 criteria to diagnose multiple sclerosis
Conclusion Our data indicate significant variation in practice amongst Scottish neurologists regarding the diagnosis of MS. It seems that few are enthusiastic about applying the current diagnostic criteria after a single attack (CIS), even if the diagnostic criteria are met. Case mix, clinical expertise and experience may explain some of this variation, and given the lack of evidence for aggressive treatment of CIS, such diagnostic caution is perhaps justified. It would be of interest to repeat the audit UK wide, and internationally, and our prediction would be that some countries may embrace the criteria with more enthusi...

The role of brain mri in the diagnosis of neuromyelitis optica
Conclusion Application of the Oxford 2012 diagnostic criteria in a clinical setting resulted in an 85% specificity for differentiating NMO from RRMS, the most common differential diagnosis compared to only 67% for Barkhof's criteria. This data should be considered in the MRI assessment of patients in whom the diagnosis of NMO is being considered. In particular patients fulfilling Barkhof's criteria should also be assessed with the Oxford 2012 criteria to improve the interpretation of brain MR imaging and its diagnostic accuracy.

A fatal case of 'primary' headache
We describe a case of a 33 year old caucasian male with a subacute history of headaches presenting to the medical team with pyrexia, confusion and hallucinations. He subsequently became disorientated, agitated and initially needed sedation and ventilation after becoming acutely hypoxic with autonomic instability. He was subsequently found to have dense, pre–geniculate blindness and generalised arreflexia. Magnetic resonance imaging demonstrated normal brain parenchyma but diffuse nodular thickening and enhancement of the basal leptomeninges in the brain and throughout the entire spinal cord. Lumbar puncture revealed ...

Sodium accumulation is associated with disability and progression in multiple sclerosis: a 23na mri study
Conclusions Significant increases in sodium were seen in lesions and normal appearing brain tissues in MS. Increased concentration of sodium in lesions, cortical grey matter, NAWM and basal ganglia in SPMS versus RRMS indicates greater neuroaxonal metabolic dysfunction and/or loss in the former group. MRI measurement of sodium concentration in vivo is likely to reflect clinically relevant neuroaxonal pathophysiology and may be a useful outcome measure in trials of putative neuroprotective treatments.

Neurological manifestations of influenza infection in adults and children: results of a national british surveillance study
Conclusion This paediatric and adult UK cohort identified a severity of influenza related neurological manifestation not reported previously. Cases were more common in children, particularly those with underlying neurological conditions. Encephalopathy syndromes such as ANE, AIEF, HSE and AHL were seen more frequently in children and were associated with a worse outcome. Acute movement disorders and Guillain–Barré syndrome were identified more commonly in adults. Influenza related encephalopathy may be more common in those with abnormal genetically determined host inflammatory responses, but the virus itself i...
