Metal Allergy Testing in Patients Listed for Temporomandibular Joint Replacement at Massachusetts General Hospital
Patients with temporomandibular joint (TMJ) arthropathies (i.e end-stage degeneration, invasive pathology, or developmental anomalies) may be candidates for total joint replacement (TJR). There is only one FDA-approved manufacturer of patient-specific alloplasts of the TMJ, Stryker-TMJ Concepts. Stryker can fabricate a custom prosthesis composed of a cobalt-chromium-molybdenum alloy, with a maximum of 1% nickel present in the condylar component. However, metal hypersensitivity can occur, which may cause failure in up to 3% of TMJ replacements.

Gonzalez-Perez et al state that an epicutaneous patch test for metal allergy ( “chrome cobalt molybdenum Co-Cr-Mb” – the correct chemical symbol for molybdenum is “Mo”), T.R.U.E. test® (SmartPractice Denmark ApS) was administered preoperatively.1 Presumably, but not stated clearly, this testing was administered to all patients before they had initial temporomandibul ar joint (TMJ) replacements using either the Christensen (CS), or the Biomet® systems (BS).

Allergic reaction to Nickel and Titanium In patients with TMJ replacements: 2 case reports
Introduction Previous retropsective and prospective studies in the TMJ and orthopaedic literature have shown a high incidence (38-42%)12,3 of allergy to metals found in implant alloys in particular Nickel, Chrome and Cobalt. Incidence of metal allergy appears to be higher in patients who have had previous prostheses especially if a metal on metal prosthesis was used 3 Patch testing has been recommended in the UK before TMJ replacement and Titianium implants advised where allergy exists.

A prospective comparison study of 1 year outcomes for all-titanium versus cobalt–chromium total temporomandibular joint replacement in metal-allergic patients
