Bilateral upper lobe Pneumocystis pneumonia during aerosolized pentamidine prophylaxis
A 37-year-old man with human immunodeficiency virus (HIV) infection presented to our hospital due to a 9-day history of fever, cough and dyspnea on exertion. He was diagnosed with HIV infection 6  years before the admission. However, combination antiretroviral therapy (cART) was only administered 9 days before admission because a previous physician could not obtain the patient’s consent. He was receiving aerosolized pentamidine, rather than trimethoprim-sulfamethoxazole, for 3 months as aPneumocystis prophylaxis because of his prior sulfa allergy. Clinical examinations revealed high-grade fever, tachycardia and res...

Traffic-Related Air Pollution and All-Cause Mortality during Tuberculosis Treatment in California
Conclusions: Residential proximity to road traffic volumes and traffic density were associated with increased all-cause mortality in patients undergoing treatment for active tuberculosis even after adjusting for multiple demographic, socioeconomic, and clinical factors, suggesting that TB patients are susceptible to the adverse health effects of traffic-related air pollution. https://doi.org/10.1289/EHP1699 Received: 31 January 2017 Revised: 18 August 2017 Accepted: 23 August 2017 Published: 29 September 2017 Address correspondence to R.J. Dilemma of managing asymptomatic children referred with 'culture-confirmed drug-resistant tuberculosis
Conclusions Bacteriological evaluation should not be performed in the absence of any clinical indication. If drug-resistant Mycobacterium tuberculosis is detected in an asymptomatic child with a normal chest radiograph, close observation may be an appropriate strategy, especially in settings where potential laboratory error and poor record keeping are constant challenges.

Unusual chest radiographic findings in children
A 9-year-old British Asian girl with asthma was referred by her general practitioner to outpatients with a 12-month history of dry cough and an abnormal chest radiograph. There was no history of recent travel, tuberculosis (TB) exposure or clinical features to suggest TB such as weight loss and night sweating. She received courses of antibiotics without improvement. She was fully immunised including BCG vaccine. She had chickenpox (varicella) infection aged 3 years. On assessment, she was found to have worsening nocturnal cough, frequent use of inhaled salbutamol, and dyspnoea, wheeze and cough on exertion. Systemic e...

Head-to-head comparison of subcutaneous abatacept versus adalimumab for rheumatoid arthritis: two-year efficacy and safety findings from AMPLE trial
Conclusions Through 2 years of blinded treatment in this first head-to-head study between biologic disease-modifying antirheumatic drugs in RA patients with an inadequate response to MTX, subcutaneous abatacept and adalimumab were similarly efficacious based on clinical, functional and radiographic outcomes. Overall, AE frequency was similar in both groups but there were less discontinuations due to AEs, SAEs, serious infections and fewer local ISRs with abatacept. ClinicalTrials.gov Identifier NCT00929864.

What Causes Spontaneous Pneumothorax?
Discussion “A pneumothorax is a collection of air in the pleural space, and it can be categorized into spontaneous, traumatic or iatrogenic. Spontaneous pneumothorax can be further classified into primary with no clinical evidence of underlying lung disease or secondary due to pre-existing lung disease.” Spontaneous pneumothorax is a condition that is relatively rare in pediatrics. There is a bimodal age distribution – neonates and late adolescence. It is caused by tearing of the visceral pleural. Clinical signs include chest pain, dyspnea, tachycardia, tracheal deviation towards contralateral side, hypot...
