A systematic review of neuroprotective strategies after cardiac arrest: from bench to bedside (part II-comprehensive protection)
Abstract Neurocognitive deficits remain a significant source of morbidity in survivors of cardiac arrest. We conducted a literature review of treatment protocols designed to evaluate neurologic outcome and survival following global cerebral ischemia associated with cardiac arrest. The search was limited to investigational therapies that were implemented either during cardiopulmonary resuscitation or after return of spontaneous circulation in studies that included assessment of impact on neurologic outcome. Given that complex pathophysiology underlies global brain hypoxic ischemia following cardiac arrest, neuropr...

Analysis of a Guideline-Derived Resident Educational Program on Inpatient Glycemic Control.
CONCLUSIONS: Our resident educational program significantly increased the number of patients receiving guideline-based inpatient insulin therapy and was associated with a reduction in mean FSG and LOS. Rates of hypoglycemia showed a statistically significant increase, whereas rates of severe hypoglycemia did not. Larger multicenter studies with adjustment for potential confounders are needed to further assess the impact of educational interventions on inpatient glycemic control. Impact of Frequent and Persistent Use of Continuous Glucose Monitoring (CGM) on Hypoglycemia Fear, Frequency of Emergency Medical Treatment, and SMBG Frequency After One Year
Conclusions: "Almost daily" use of CGM with the Dexcom G4 system reduced incidence of emergency treatment events and daily SMBG utilization among survey respondents and a trend toward reduced hypoglycemia fear. This may indicate cost savings in reduction of emergency medical intervention and likely improved quality of life without increasing safety concerns related to hypoglycemia.

Risk factors of severe hypoglycemia requiring medical assistance and neurological sequelae in patients with diabetes: A case–control study
Abstract: Hypoglycemia commonly occurs in patients who are being treated for diabetes. In some cases, these patients suffer from severe hypoglycemia that requires medical assistance and which can unfortunately result in long-term disabilities. Therefore, we investigated risk factors associated with severe hypoglycemia requiring medical assistance (HMA) and the resulting neurological sequelae in patients with diabetes. This investigation was a case–control study that assessed 129 patients with diabetes and documented hypoglycemia from a single tertiary hospital between February 2013 and May 2015. They were treated with or...

Agencies Pivot Patient Care Response Resources amid the COVID-19 Pandemic
A mobile integrated healthcare (MIH) program in Sacramento County (CA) under development to be implemented as an alternative model for response to low-acuity calls and high utilizers of emergency services was quickly brought to an operational status. It was then redesigned and redirected to serve a new purpose as the COVID-19 pandemic struck northern California and challenged patients’ access to appropriate healthcare. Upon the outbreak of COVID-19, three Sacramento-area fire departments joined forces with local hospitals to rapidly mobilize a MIH program to deliver patient-centric care, testing and education to out-o...

SEE-Diabetes, a patient-centered diabetes self-management education and support for older adults: Findings and information needs from patients' perspectives
CONCLUSIONS: We found the high use of patient portal for reading clinic notes, which offers a use case opportunity for the proposed SEE-Diabetes educational aid. Feedback about the readability and helpfulness of clinic notes will be considered during the design process.PMID:35227635 | DOI:10.1016/j.pcd.2022.02.008

Position paper of the Italian Association of Medical Diabetologists (AMD), Italian Society of Diabetology (SID), and the Italian Study Group of Diabetes in pregnancy: Metformin use in pregnancy
ConclusionsMetformin may represent a therapeutic option in selected women with obesity, PCOS, GDM, T2DM, and in women undergoing ART. However, more research is required specifically on the long-term effects of in utero exposition to metformin.
