To mix or not to mix in allergy immunotherapy vaccines
Purpose of review European and US allergists generally do not agree on the approach to subcutaneous allergy immunotherapy in patients with multiple allergies. The North American approach is to treat with a mixture that contains all the allergen extracts to which the patient has evident clinical sensitivity, whereas the European approach is to select for treatment the one or at the most two allergens that are clinically most important for the patient. Social determinants of health in the world of allergy/immunology
This month's Annals focuses on the social determinants of health. This topic has received an even greater interest since the coronavirus disease 2019 (COVID-19) pandemic, in which tremendous differences in vaccination and mortality rates have been seen among different ethnic groups within the United States. However, even before the pandemic, there have been significant disparities because of social determinants. In the case of allergy/immunology (A/I), as noted in the review by Grant et  al,1 most data regarding this matter are focused on asthma.

The United States Drug Allergy Registry: A Prospective Longitudinal Cohort Study of Patients Tested for Drug Allergies
Drug allergy mislabeling negatively impacts clinical care and public health while preventing scientific advancements related to drug hypersensitivity. Given that single site US studies lack power and generalizability, we aimed to assemble the first multi-site US prospective longitudinal cohort of patients tested for drug allergies.

Don't Hold the Tahini: A Retrospective Chart Review of Sesame Oral Food Challenges in a Pediatric Food Allergy Clinic
Although allergy to sesame has a growing prevalence in the United States and is a common cause of anaphylaxis worldwide, diagnostic predictors of clinical reactivity remain elusive. Measures of sensitization including skin prick testing (SPT) and specific IgE (sIgE) have been poor predictors of outcomes of the gold standard oral food challenge (OFC). Here we report the results of sesame OFCs in a pediatric clinic population.

Delay in peanut allergy diagnosis in the United States
Most commonly, food allergy (FA) symptoms present within the first few years of life. However, these initial concerns may not coincide with the time of an official diagnosis by an experienced healthcare provider (HCP). Potential delays in diagnosis (DD) can result in unwarranted changes in lifestyle or continued accidental exposures and reactions. The purpose of this retrospective study is to determine the delay in FA diagnosis from first reaction and evaluate the factors that may affect this time period in peanut-allergic children.

Prevalence and Phenotype of Food Allergy in Cystic Fibrosis
Cystic Fibrosis (CF) patients may present with food-associated gastrointestinal symptoms similar to those seen in food allergy (FA). Minimizing food restrictions is a priority for these patients to optimize overall nutrition. Pomegranate seed allergy in a child with multiple tree nut allergies
Pomegranate (Punica granatum) is a fruit-bearing plant of the Lythraceae family that consists of pulp surrounding fleshy seeds. Pomegranate juice and seeds are widely consumed, and rare cases of hypersensitivity reactions to pomegranate have been described in other parts of the world. Here, we report the first case of IgE-mediated pomegranate seed allergy in a tree nut-allergic child in the United States.

Is it time to set the alarmins as potential targets in food allergy?
Food allergies affect an estimated 8% of children, and up to 10% of adults in the United States, and prevalence is increasing over time. 1,2 While primary prevention through early exposure is of utmost importance, there is a substantial need for disease-modifying therapies. Food allergy treatment is an active area of research, particularly in the last several years, leading to the FDA-approval of the first product for this purpose, Palforzia (Aimmune Therapeutics, Brisbane, CA), for peanut oral immunotherapy (OIT) in 2020.
