Conclusions: There is an unexplained incidence (3.6%) of early valve degeneration resulting in explantation in some patients at two years after surgery. Mitroflow valves may not be suited to a younger age population.

Left Atrial Reverse Remodeling Following Valve Surgery for Chronic Degenerative Mitral Regurgitation in Patients with Preoperative Sinus Rhythm: Effects on Long‐Term Outcome
ConclusionsPreoperative LA enlargement due to severe degenerative MR in patients with sinus rhythm demonstrates a high potential for postoperative reverse remodeling following MVS. The absence of postoperative LARR was not associated with an increase in the risk of postoperative mortality or adverse clinical events.

Successful closure of residual leak following LARIAT procedure in a patient with high risk of stroke and hemorrhage.
Abstract Percutaneous left atrial appendage suture ligation with the LARIAT® device (Sentre HEART, Redwood City, California) was successfully performed on an 84‐year‐old woman with non‐valvular atrial fibrillation who developed intracranial hemorrhage on warfarin. First in‐vivo evaluation of a flexible self‐apposing left atrial appendage closure device in the canine model
Conclusion: The percutaneous delivery of a novel self‐positioning LAA occlusion device is feasible and safe in a canine model. At 30 days, all devices displayed complete healing and occlusion of the LAA without any device related adverse events. © 2013 Wiley Periodicals, Inc.

Thromboembolism in a patient with a mechanical mitral valve during anticoagulation with dabigatran etexilate.

Endovascular versus open elephant trunk completion for extensive aortic disease
Objectives: To compare the outcomes between patients undergoing endovascular (EEC) or open (OEC) approaches to second-stage elephant trunk completion (EC).Methods: From 1993 to 2010, 225 patients underwent second-stage EC (EEC, n = 92; OEC, n = 133). Propensity matching was performed for a fair comparison.Results: The EEC patients were older, more likely to have atrial fibrillation, and had a smaller proximal aorta. Surgical Treatment of Atrial Fibrillation: Today’s Questions and Answers
Although atrial fibrillation is common in cardiac surgery patients, the Cox maze IV procedure is underutilized; in contemporary practice, most cardiac surgery patients with atrial fibrillation do not receive concomitant surgical ablation. Available evidence suggests that a biatrial, energy-assisted Cox maze IV procedure restores normal sinus rhythm in two-thirds to three-quarters of patients without increasing operative risk. Reoperative aortic valve replacement in the octogenarians—minimally invasive technique in the era of transcatheter valve replacement
Conclusions: Octogenarians who undergo re-AVR are thought to be high-risk surgical candidates. The present single-center series revealed acceptable in-hospital outcomes and operative mortality. Mre-AVR was associated with better survival compared with Fre-AVR and might benefit this population.

Guideline adherence in antithrombotic treatment after concomitant ablation surgery in atrial fibrillation patients
CONCLUSIONS Real-life OAC prescription in AF patients showed a moderate guideline adherence, with high-risk patients being undertreated and low-risk patients being overtreated. These findings stress the importance that antithrombotic treatment in patients undergoing AF surgery needs to be critically re-evaluated.

Two-staged hybrid treatment of persistent atrial fibrillation: short-term single-centre results
CONCLUSIONS The sequential, two-staged hybrid strategy (surgical thoracoscopic followed by catheter ablation) is feasible and safe with a high post-procedural success and seems to represent the optimal treatment with low risk load and potentially long-term benefit for patients with a persistent and long-standing persistent form atrial fibrillation.
