Right anterior minithoracotomy versus conventional aortic valve replacement: A propensity score matched study
Conclusions: Right anterior minithoracotomy in patients undergoing isolated aortic valve surgery is associated with a lower incidence of postoperative atrial fibrillation and blood transfusion and shorter ventilation time and hospital length of stay. Prospective randomized trials are needed to confirm our data.

Abstract 227: Prescribing Warfarin at Discharge for Heart Failure Patients without Atrial Fibrillation Findings from the Get With The Guidelines-Heart Failure Registry Poster Session II
Conclusions: Approximately 1 in 10 HF patients without AF or other documented indications are prescribed warfarin at discharge, and prescription rates vary widely across hospitals. Further research is needed to understand why anticoagulation is prescribed to patients with HF in the absence of documented indications.

Variation in Warfarin Thromboprophylaxis After Mitral Valve Repair: Does Equipoise Exist and Is a Randomized Trial Warranted?
CONCLUSIONS: At present, half of patients are prescribed warfarin after isolated mitral valve repair in North American cardiac surgical practice which may impact the length of hospital stay. Although patient-level predictors of warfarin prescription exist, center- and surgeon-level variations are prominent. There is a pressing need for a randomized trial both to guide therapy and to ascertain the potential for resource conservation. PMID: 23706423 [PubMed - as supplied by publisher]

Ringless alfieri mitral valve repair for significant ischemic mitral regurgitation with coronary artery bypass grafting.
Conclusions: Alfieri mitral valve repair is associated with lower risks of mortality, postoperative stroke, and prolonged intensive care unit and hospital stays. Alfieri mitral valve repair and concomitant CABG surgery can be performed in patients with IMR. PMID: 23958527 [PubMed - in process]

065 * a propensity-matched analysis of outcomes and long-term survival in stented versus stentless valves for aortic valve replacement
Conclusions: Stentless and stented valves had identical 30-day outcomes except for a higher incidence of postoperative bleeding in the stented group. There was no significant difference in long-term survival between stented and stentless valves in this large cohort. Both valves may be used for AVR with low morbidity and excellent long-term survival.

275 * early prosthetic valve degeneration with mitroflow aortic valves: determination of incidence and risk factors
Conclusions: There is an unexplained incidence (3.6%) of early valve degeneration resulting in explantation in some patients at two years after surgery. Mitroflow valves may not be suited to a younger age population.

Statin therapy prior to CABG surgery may improve outcomes
Patients receiving statin therapy before coronary artery bypass grafting (CABG) surgery appear to have a reduced risk of post-surgical mortality, stroke, and atrial fibrillation (irregular or rapid heart rate), according to an article in the October 2013 issue of The Annals of Thoracic Surgery. These same benefits from statins could not be demonstrated for patients undergoing aortic valve replacement (AVR). "Heart surgery patients typically have a number of other disorders, so we need to optimize the patient's preoperative condition and help ensure the best possible result," said Elmar W...

Cardiac Critical Care After Transcatheter Aortic Valve Replacement
Transcatheter aortic valve replacement (TAVR) is a new therapy for severe aortic stenosis now available in the United States. Initial patients eligible for TAVR are defined by high operative risk, with advanced age and multiple comorbidities. Following TAVR, patients experience acute hemodynamic changes and several possible complications, including hypotension, vascular injury, anemia, stroke, new-onset atrial fibrillation, conduction disturbances and kidney injury, requiring an acute phase of intensive care. Alongside improvements in TAVR technology and technique, improvements in care after TAVR may contribute to improved...

Thromboembolism in a patient with a mechanical mitral valve during anticoagulation with dabigatran etexilate.
We report a 57-year-old man with a mechanical heart valve who experienced acute upper limb thromboembolism during dabigatran intake. Dabigatran might be inadequate for thromboprophylaxis after mechanical valve replacement. PMID: 24182477 [PubMed - in process]

Reoperative aortic valve replacement in the octogenarians—minimally invasive technique in the era of transcatheter valve replacement
Conclusions: Octogenarians who undergo re-AVR are thought to be high-risk surgical candidates. The present single-center series revealed acceptable in-hospital outcomes and operative mortality. Mre-AVR was associated with better survival compared with Fre-AVR and might benefit this population.

Ascending-to-Descending Aoritc Bypass: A Simple Solution to a Complex Problem.
CONCLUSIONS: The ascending-to-descending aortic bypass can be performed with low morbidity and mortality. It is an effective solution to complex aortic coarctation and represents a safe single-stage approach for patients with concomitant cardiac pathology.
