Poststroke dementia is associated with recurrent ischaemic stroke
Conclusions Poststroke dementia predicts recurrence of ischaemic stroke in long-term follow-up and should be considered when estimating prognosis.

Socioeconomic deprivation and provision of acute and long-term care after stroke: the South London Stroke Register cohort study
Conclusions Stroke healthcare inequalities in England exist for some important indicators, although overall it has improved over time. The impact of SED may be stronger in black patients than in white patients. Further efforts are required to achieve stroke care equality.

General medical skills on neurology and stroke wards
This study looks at the difference in the general medical care required in neurology and stroke units and the different skill set required. Methods Prospective data regarding diagnosis and medical complications were collected from 100 patients admitted to a neurology and a stroke ward. Results Stroke unit patients were older (mean age 71.4 yrs; standard deviation 16.3) than those on the neurology ward (47.9;16.8, p=<0.001). Complexity of heart rate variability predicts outcome in intensive care unit admitted patients with acute stroke

Newly detected paroxysmal atrial fibrillation after tia and ischaemic stroke on cardiac monitoring: systematic review and meta-analysis in relation to duration of recording
Conclusion Cardiac monitoring after TIA or ischaemic stroke detects clinically important rates of pAF in studies of unselected populations, with high rates of subsequent anticoagulation. A monitoring period of 5–7 days appears to be adequate.

Incidence, outcome and future projections of atrial fibrillation-related stroke and systemic embolism at age >=80 years: 10-year results of a population-based study
Conclusion Numbers of AF–related ischaemic strokes at age ≥80 years are projected to treble by 2050, along with numbers of systemic emboli, unless rates of anticoagulation in older patients increase.

Recent time trends in incidence, outcome and premorbid treatment of atrial fibrillation-related stroke and other embolic vascular events: a population-based study
Conclusions We found no reduction in incidence of AF-related vascular events since publication of the BAFTA trial. A third of all disabling/fatal strokes occur in non-anticoagulated patients with known prior AF.

Aetiology, secondary prevention strategies and outcomes of ischaemic stroke despite oral anticoagulant therapy in patients with atrial fibrillation
Conclusions Stroke despite anticoagulation comprises heterogeneous aetiologies and cardioembolism despite sufficient anticoagulation is most common. While DOAC were associated with better outcomes than VKA, adding antiplatelets was linked to worse outcomes in these high-risk patients. Our findings indicate that individualised and novel preventive strategies beyond the currently available anticoagulants are needed. Trial registration number ISRCTN48292829.

Early versus late start of direct oral anticoagulants after acute ischaemic stroke linked to atrial fibrillation: an observational study and individual patient data pooled analysis
Conclusions Our results do not corroborate concerns that an early DOAC-start might excessively increase the risk of ICH. The sevenfold higher risk of recurrent AIS than ICH suggests that an early DOAC-start might be reasonable, supporting enrolment into randomised trials comparing an early versus late DOAC-start.

Conjugate upward gaze paralysis with unilateral ptosis caused by a unilateral midbrain infarction
A 73-year-old woman with atrial fibrillation presented with a sudden right hemiparesis, with diplopia and left ptosis, and was admitted at an Emergency Unit. The neurological examination found fluctuations on consciousness level, predominant crural right hemiparesis and right central facial paralysis without sensitive abnormalities. Oral anticoagulant prescribing in a hyperacute stroke unit

Acute hemifield room tilt illusion in cerebral ischaemia
We present 2 cases in the setting of acute vertebrobasilar territory ischaemia. Case 1: A 56-year old man was admitted with acute dyspnoea and new atrial fibrillation.
