A traveler in transit: A case of an impending thrombus entrapped in a patent foramen ovale
We report a case of a 50 ‐year‐old male taxi driver who was diagnosed with a massive saddle pulmonary embolism, leg deep venous thromboembolism, and pending paradoxical embolus through a PFO with systemic embolization. The patient had an inferior vena cava (IVC) filter inserted immediately followed by surgical thromboem bolectomy and closure of PFO. He was discharged home 1 month after surgery. Surgery is the treatment of choice for preventing systemic embolization particularly cryptogenic stroke and its sequelae from pending paradoxical embolus. Final results of the Randomised Evaluation of short-term DUal antiplatelet therapy in patients with acute Coronary syndromE treated with a new generation stent (REDUCE) trial.
CONCLUSIONS: The results show that among ACS patients treated with COMBO, 3 months is non-inferior to 12 months DAPT. However, given the numerically higher rates of mortality and ST in the 3 months DAPT group, 1-year DAPT should still be recommended in ACS until more information becomes available. ConclusionsWe established a novel formula to mathematically calculate actual 3D angles between inflow cannula and device body of HeartMate II. The formula would help investigators to validate their findings of the relationship between 2D projected angle (from CXR) and device thrombosis.

Reversal and Resumption of Antithrombotic Therapy in LVAD-Associated Intracranial Hemorrhage
ConclusionsReversal of anticoagulation appears safe after LVAD-associated intracranial hemorrhage, though inadequate reversal was common. Resumption of warfarin ± antiplatelet was associated with fewer fatal and nonfatal thrombotic events compared with antiplatelets alone, though more nonfatal hemorrhage events occurred.

Commentary: Postoperative atrial fibrillation can last years? Oh snap!
Despite improvements in perioperative and operative care, postoperative atrial fibrillation (POAF) is still found in approximately 15% to 60% of cardiac surgical patients.1 The potential clinical impact on patients is dependent on mainly their clinical status and can include heart failure, acute renal failure, stroke or thromboembolism, anticoagulation-related complications, and even death. This is particularly true because a significant proportion of patients with aortic stenosis have diastolic dysfunction and rely on the atrial contraction to maintain adequate cardiac output.

Outcomes after Ross procedure in adult patients: A meta ‐analysis and microsimulation
ConclusionsRoss procedure in nonelderly adults is associated with low mortality and low risk of adverse events both at short ‐ and long‐term follow‐up. The surgical community must prioritize a large, expertize‐based randomized controlled trial to definitively address the risks and benefits of the Ross procedure compared to conventional aortic valve replacement.

Determining Which Prosthetic to Use During Aortic Valve Replacement in Patients Aged Younger than 70 Years: A Systematic Review of the Literature.
CONCLUSIONS: Published literature does not preclude the use of bioprostheses for AVR in younger patients. As new valves are developed, the use of bioprosthetic aortic valves in younger patients will likely continue to expand. Clinical trials are needed to provide surgeons with more accurate guidelines.
