Long-term Outcomes After Thrombolytic Therapy for Acute Ischemic Stroke
The role of intravenous thrombolytic therapy in the management of acute ischemic stroke is well established, and faster administration of intravenous tissue plasminogen activator (tPA) has been associated with better short-term outcomes in clinical practice.

Intravenous Thrombolysis Before Endovascular Thrombectomy for Acute Ischemic Stroke
The 2 reperfusion therapies of proven benefit for acute ischemic stroke, mechanical endovascular thrombectomy (EVT) and pharmacologic intravenous thrombolysis (IVT), have complementary advantages. EVT, which involves mechanical debulking, works well for accessible sizeable thrombi that occlude large cerebral vessels and are resistant to rapid thrombolytic dissolution. IVT, which involves chemical dissolution, works well for smaller thrombi that occlude medium and small cerebral vessels inaccessible or poorly accessible to endovascular technology. An open, important question has been whether the modest efficacy of IVT for l...

Ipsilateral Vocal Cord Paralysis After Acute Anterior Ischemic Stroke
CONCLUSION: Our case is a rare case of transient ipsilateral vocal cord paralysis associated with anterior unilateral cerebral ischemia. The paralysis resolved with improvement of the cerebral ischemic penumbra.PMID:33635769 | DOI:10.7812/TPP/20.104

Preliminary Investigation of In situ Thrombus Within Patent Foramen Ovale in Patients With and Without Stroke
This study uses high-resolution optical coherence tomography to investigate whether patent foramen ovale could be the site of thrombus formation in patients with or without stroke.

A case of “smoldering” immune‐mediated thrombotic thrombocytopenic purpura manifesting as recurrent cardioembolic stroke
We report an atypical manifestation of immune-mediated thrombotic thrombocytopenic purpura (TTP) that we hypothesize represents a longstanding, or “smoldering,” process. As typified by this case, the prompt recognition and treatment for immune-mediated TTP are often confounded by clinical comorbidities. Thus, TTP should be considered in cases of unremitting thrombocytopenia of unknown origin in patients with stroke or other thrombosis. AbstractPrompt recognition and treatment for thrombotic thrombocytopenic purpura (TTP) are critical to prevent the irreversible manifestations of this rare and quickly fatal hematologic ...

Statistical analysis plan for the randomized controlled trial Tenecteplase in Wake-up Ischaemic Stroke Trial (TWIST)
Patients with wake-up ischemic stroke are frequently excluded from thrombolytic treatment due to unknown symptom onset time and limited availability of advanced imaging modalities. The Tenecteplase in Wake-up ...

AstraZeneca COVID ‐19 vaccine: A possible risk factor for ischemic stroke and cerebral venous sagittal sinus thrombosis: A case series
AbstractOne of the most prevalent neurological impairments is cerebrovascular accident (CVA). Ischemic stroke and CVST have been linked to the AstraZeneca COVID-19 vaccine. Three Sudanese patients developed these diseases after receiving the AstraZeneca COVID-19 vaccine, indicating a relationship between the AstraZeneca COVID-19 vaccine and these conditions.

Congenital absence of left atrial appendage in a patient with ischemic stroke and atrial fibrillation
AbstractLeft atrial appendage (LAA) is a finger-like muscular extension of the left atrium, and it is the most prominent site for cardiac thrombus in patients with atrial fibrillation. Congenital absence of LAA could be incidentally detected in patient with ischemic stroke and atrial fibrillation. Although it is considered to be an extremely rare cardiac anomaly, its clinical significance remains unknown and there is no clear consensus in the management strategy in those patients. Therefore, we report a case of an incidentally noted congenital absence of LAA in a 68-year-old woman being planned for LAA closure.

Different types of percutaneous endovascular interventions for acute ischemic stroke
CONCLUSIONS: This review did not establish any difference in safety and effectiveness between the thrombo-aspiration approach and stent-retrieval thrombectomy for treating people with AIS. Furthermore, the combined group did not show any obvious advantage over either intervention applied alone.PMID:37249304 | DOI:10.1002/14651858.CD014676.pub2

A case of Lemierre's syndrome causing panhypopituitarism, cavernous sinus thrombosis, ischaemic stroke and pulmonary embolism
We present the case of a previously well 19-year-old man, who presented acutely unwell with meningitis and sepsis. Fusobacterium necrophorum was isolated from peripheral blood cultures and identified on cerebrospinal fluid with 16S rDNA Polymerase Chain Reaction (PCR). Imaging demonstrated internal jugular vein thrombosis with subsequent cavernous venous sinus thrombosis. Pituitary function tests were suggestive of panhypopituitarism. The patient was diagnosed with Lemierre's syndrome complicated by meningitis, cavernous sinus thrombosis, base of skull osteomyelitis, ischaemic stroke and panhypopituitarism. He was treated ...
