Role of Dexmedetomidine in Aneurysmal Subarachnoid Hemorrhage: A Comprehensive Scoping Review
Dexmedetomidine (DEX), an α2-adrenergic agonist, has been widely used for anesthesia, pain control, and intensive care unit sedation. Besides sleep-like sedation, DEX has many other beneficial effects, such as anti-inflammation, antioxidation, and anticell death. Subarachnoid hemorrhage (SAH), a severe and potentially fatal form of stroke, is a complex disease that is divided into 2 phases: early brain injury and delayed cerebral ischemia. In each phase, several pathologic changes are involved, including disturbed intracranial homeostasis, metabolic failure, blood-brain barrier damage, vasospasm, microthrombosis, and cort...

Safety of SARS-CoV-2 vaccination in patients with Behcet's syndrome and familial Mediterranean fever: a cross-sectional comparative study on the effects of M-RNA based and inactivated vaccine
Rheumatol Int. 2022 Apr 4. doi: 10.1007/s00296-022-05119-y. Online ahead of print.ABSTRACTMost of the published data relate to classical forms of rheumatic diseases (RD) and information on rare inflammatory disorders such as Behçet's syndrome (BS) and familial Mediterranean fever (FMF) is limited. We studied the frequency of side effects and disease flares after COVID-19 vaccination with either Pfizer/BioNTech or Sinovac/CoronaVac in 256 patients with BS, 247 with FMF, and 601 with RD. Telephone interviews were conducted using a questionnaire survey in a cross-sectional design in patients with BS, FMF, and RD followed by ...

Assessment of Perioperative Outcomes Among Surgeons Who Operated the Night Before
CONCLUSIONS AND RELEVANCE: The findings of this cross-sectional study suggest that operating overnight was not associated with worse outcomes for operations performed by surgeons the subsequent day. These results provide reassurance concerning the practice of having attending surgeons take overnight call and still perform operations the following morning.PMID:35604661 | DOI:10.1001/jamainternmed.2022.1563

Pharmacological inactivation of the primate posterior insular/secondary somatosensory cortices attenuates thermal hyperalgesia
ConclusionsOur data emphasize that increased activity in the PIC/SII after appearance of thalamic lesions can contribute to abnormal pain of multiple modalities, and the modulation of PIC/SII activity may be a therapeutic approach for thermal hyperalgesia.

Male Sexual Dysfunction
Health Psychol Res. 2022 Aug 20;10(3):37533. doi: 10.52965/001c.37533. eCollection 2022.ABSTRACTMale sexual dysfunction is a series of conditions, most notably including erectile dysfunction (ED), Peyronie's disease (PD), and premature ejaculation (PE), defined by impaired sexual functioning. The prevalence of male sexual dysfunction increases with age and is relatively high with greater than 50% of men aged 40 to 70 describing some degree of erectile dysfunction. Risk factors for male sexual dysfunction include age, diabetes mellitus (DM), cancer, stroke, hypertension, penile trauma, depression, anxiety, and disturbance i...

A Case of Acute Cerebral Infarction With Chief Complaints of Abdominal Pain and Bloody Diarrhoea: The Power of a Patient-Centered Inclusive Diagnostic Team
We present the case of a 66-year-old man who presented with acute abdominal pain and bloody stool as his chief complaints and was finally diagnosed with ischemic colitis from colon cancer and acute cerebral infarction. Although several cognitive biases led to physicians missing the presence of acute stroke, a diagnostic team consisting of the patient, his family members, a ward nurse, and the physician worked effectively to reach the correct diagnosis soon after admission. A physician is not the only person involved in the diagnostic process. A patient-centered diagnostic team is necessary.PMID:36046325 | PMC:PMC9418667 | ...

Opioid Free Cardiac Surgery: A Multimodal Pain Management Strategy with a Focus on Bilateral Erector Spinae Plane Block Catheters
Cardiac surgery requiring sternotomy presents unique challenges for perioperative and postoperative analgesia. Intraoperatively, the sympathetic response to surgical stimulation must be carefully modulated to prevent excessive bleeding, maintain the integrity of surgical repairs, and decrease the risk of stroke or iatrogenic aortic injury without compromising myocardial and systemic perfusion. This must be achieved in patients whose underlying abnormal physiology coupled with the effects of the surgical procedure often result in tenuous hemodynamics.

Can ED chest pain patients with intermediate HEART scores be managed as outpatients?
CONCLUSION: This study demonstrates that rapid access clinics are likely a safe alternative to admission for intermediate-risk chest pain patients and could reduce costly inpatient admissions for chest pain. With angiography excluded, MACE rates were well below the American College of Emergency Physicians cited 2% threshold.PMID:36129627 | DOI:10.1007/s43678-022-00355-4
