Decompressive hemicraniectomy for malignant middle cerebral artery infarction including patients with additional involvement of the anterior and/or posterior cerebral artery territory —outcome analysis and definition of prognostic factors
ConclusionsIntensified postoperative management including possible secondary decompression with necrosectomy may further reduce case fatality rate of patients with large hemispheric infarction. Age above 60  years and severely reduced level of consciousness are the most significant factors heralding unfavorable recovery. Patients suffering infarctions exceeding the MCA territory have a comparable chance of favorable recovery as patients with isolated MCA infarction.

The impact of statin therapy after surgical or endovascular treatment of cerebral aneurysms.

Factors Influencing Long-Term Blood Flow in Extracranial-to-Intracranial Bypass for Symptomatic Internal Carotid Artery Occlusive Disease: A Quantitative Study
CONCLUSION: Double anastomoses and cilostazol resulted in long-term STA blood flow preservation. No recurrence of cerebral infarction was noted in either STA hypoperfusion or occlusion cases.

266 A drowsy patient: beyond the familiar territory
A 72-year-old man was brought to the hospital after he was found unresponsive at home by his wife who reported that he has been sleepy in the last few days. He has a background of hypertension, myocardial infarct, and osteoarthritis. His medications include anti-hypertensive and morphine sulphate. The risk factors of postoperative infarction after surgical clipping of unruptured anterior communicating artery aneurysms: anatomical consideration and infarction territory
ConclusionHypertension, previous stroke, posteriorly projecting aneurysms, aneurysm size, and highly positioned aneurysms are independent risk factors for postoperative infarction during surgical clipping of an unruptured anterior communicating artery aneurysm. Additionally, a closed A2 plane is an additional risk factor of postoperative infarction in patients undergoing clipping via the pterional approach.

DiscussionThis case illustrates the important clinical aspects and physical findings needed to diagnose DRS. The presence of focal allodynia and hyperalgesia with recent cerebrovascular incidents provides significant evidence for DRS. The purpose of this case is to provide awareness and to increase clinical suspicion of DRS during examination, especially in the setting of cerebral venous sinus occlusion as infarction alone may not present as atypical allodynia.Disclosures S. E-150 Mechanical thrombectomy in distal middle cerebral artery occlusions using a novel blind exchange mini-pinning technique: experience in 7 consecutive cases
ConclusionBlind mini-pinning allows for combined use of distal stentriever with a 035 thrombectomy catheter, affording a larger ID than traditional microcatheter engagement of the thrombus with enhanced stability for removal, and improved reperfusion rates in distal occlusions. We describe a milder case of DVA thrombosis in the setting of COVID-19 infection presenting with unilateral hemiparesis requiring medical management alone. A 41-year-old female with hypertension who presented with left hemiparesis. Non-contrast computed tomography (CT) of head revealed linear calcifications in the right centrum semiovale consistent with DVA. Magnetic resonance imaging (MRI) revealed an acute ischemic stroke in the right corona radiata adjacent to the vascular abnormality. This study presents a unique case of a 54-year-old male patient with a history of stroke, liver cirrhosis, portal vein thrombosis, hypertension, diabetes, and bladder cancer, who experienced multiple episodes of intracranial hemorrhages, stroke and worsening confusion over the past five years.
