E-040 Factors associated with failed mechanical thrombectomy in acute ischemic stroke
ConclusionFailed MT was encountered in approximately 12% of cases. The most common location of failed MT was intracranial and this was predominantly due to ICAS. Further studies to evaluate better treatment modalities of ICAS related LVO are warranted.Disclosures O. Lajthia: None. H. Ali: None. R. Neyes: None. E. Almallouhi: None. R. Chalhoub: None. K. E-147 Ephrin receptor beta 1 single nucleotide polymorphisms in association with age in acute stroke patients undergoing thrombectomy
Conclusion193 patients were included in this interim analysis across 23 centers in the United States, between 02/2021 and 11/2021. For the EPHB1 SNP (rs13073658), Table 1 shows the numbers and proportions of patients with varying expressions of the SNP. While no patients 50 or older were homozygous for the SNP, 16% of those under 50 years old were. The potential relationship between EPHB1 SNP and stroke in younger patients requires further evaluation.Abstract E-147 Table 1Disclosures J. Does bariatric surgery change the risk of acute ischemic stroke in patients with a history of transient ischemic attack? A nationwide analysis
CONCLUSIONS: After analyzing nationwide information, we conclude bariatric surgery helps decrease risk of AIS in patients with a history of TIA. However, this comparison is limited by the nature of the database; further studies are needed to better understand these results.PMID:36581552 | DOI:10.1016/j.soard.2022.11.013

Choice and Timing of Antithrombotic After Ischemic Stroke, Intracerebral Hemorrhage or Cerebral Venous Thrombosis
CNS Neurol Disord Drug Targets. 2023 Mar 9. doi: 10.2174/1871527322666230309100626. Online ahead of print.ABSTRACTStroke is a multifactorial vascular disease and remains a leading cause of disability in the United States. Strokes can be ischemic or hemorrhagic in nature and secondary to arterial or venous disease, making determining the etiology and secondary prevention strategy important for preservation of the injured brain, prevention of recurrent strokes, and in the maintenance of good functional outcomes for patients impacted by stroke. In this narrative review, we provide a synopsis of the available medical evidence ...

Vagus Nerve Stimulation Paired with Upper-Limb Rehabilitation After Stroke: Two- and Three-Year Follow-up from the Pilot Study
CONCLUSIONS: Significant effects of Paired VNS therapy at one year were maintained at years 2 and 3, and further improvements in both impairment and function were observed in years 2 and 3. These changes were associated with improvements in measures of activity and participation.PMID:37001842 | DOI:10.1016/j.apmr.2023.02.012

Comprehensive Cardiac Rehabilitation for Secondary Prevention After Transient Ischemic Attack or Mild Stroke: PSYCHOLOGICAL PROFILE AND OUTCOMES
Purpose: Having previously reported that comprehensive cardiac rehabilitation (CCR) is effective for secondary prevention post-transient ischemic attack (TIA)/mild nondisabling stroke (MNDS), we present psychometric findings for the same sample that elucidate subacute TIA/MNDS psychological outcomes and test whether CCR would be independently associated with psychological improvements. Methods: In this prospective cohort trial patients with ≥1 risk factor, recruited from a stroke prevention clinic within 12 months (mean = 11.5 weeks) post-TIA/MNDS, entered CCR. Results: Of the 110 recruited patients, 100 (mean age = 65...

Stroke prevention strategies in North American patients with atrial fibrillation: the GLORIA ‐AF registry program
ConclusionsIn GLORIA‐AF, 20% of the population comprising males with CHA2DS2‐VASc ≥1 and females with CHA2DS2‐VASc ≥2 did not receive oral anticoagulation therapy. Patient characteristics associated with a lower likelihood of OAC prescription were use of antiplatelet drugs, paroxysmal pattern of AF, a history of falls, and prior bleeding.

DEFUSE-3 Trial: Reinforcing Evidence for Extended Endovascular Intervention Time Window for Ischemic Stroke
The incidence of ischemic stroke in the United States is 3.29 per 1000 persons per year, making it the first and fourth greatest causes of disability and mortality, respectively.1 Previous randomized clinical trials (RCTs) investigating acute ischemic stroke (AIS) due to large-vessel occlusion (LVO) have shown that endovascular intervention is clinically beneficial when performed within 6 hours of onset.2-7 A few of these RCTs had perfusion imaging inclusion criteria, showing superior results when compared with similar RCTs without such criteria.

The Role of Interventional Radiologists in Acute Ischemic Stroke Interventions: A Joint Position Statement from the Society of Interventional Radiology, the Cardiovascular and Interventional Radiology Society of Europe, and the Interventional Radiology Society of Australasia
Stroke is a major public health issue. Worldwide, the incidence of new strokes is 16.9 million per year (1). In the United States, the incidence of new strokes is 795,000 per year, of which 87% are ischemic (2). Endovascular thrombectomy (EVT) is proven to provide better clinical outcomes in patients with ischemic strokes caused by large-vessel occlusion compared with best medical therapy alone (3 –12). Furthermore, endovascular thrombectomy has proven to be cost-effective (13). The American Heart Association (AHA) and multiple international stroke organizations recommend EVT as standard of care for selected patients (14–18).
