Mitral Valve Blood Cyst and Multi-Territory Ischaemic Stroke
A 78-year-old male with a history of hypertension and hypercholesterolaemia was admitted to hospital with acute left eye visual loss and diagnosed with retinal artery occlusion. Magnetic resonance imaging (MRI) revealed multiple, bilateral cerebral infarcts involving supra- and infra-tentorial brain consistent with acute ischaemic stroke from a central embolic source. Extended ambulatory electrocardiographic monitoring displayed sinus rhythm with no evidence of atrial flutter or fibrillation. Transthoracic echocardiography (TTE) revealed a cystic structure associated with the mitral valve, with no restriction or regurgitation.

Differential association between apolipoprotein B and LDL cholesterol and cerebral atherosclerosis according to pre-stroke statin use
To reduce cardiovascular risk, low-density lipoprotein cholesterol (LDL-C) is the primary target of statin treatment, while apolipoprotein B (ApoB) is secondary. We investigated the association between atherosclerotic stenosis and LDL-C or ApoB levels and whether a difference in association exists according to pre-admission statin use in ischemic stroke patients.

Remnant cholesterol (RC), a non-traditional previously neglected risk factor for CVD, has received much attention in recent years. Correspondence to: “Association of remnant cholesterol with risk of cardiovascular disease events, stroke, and mortality: A systemic review and meta-analysis”
We read with great interest the paper by Yang et al. published in Atherosclerosis, reporting that remnant cholesterol (RC) levels are associated with increased risk of adverse cardiovascular events, stroke and mortality [1]. Given the recent advances in hypolipidemic treatments, RC has been brought to the spotlight as a potential prognostic and therapeutic biomarker of residual atherosclerotic cardiovascular disease risk (ASCVD) beyond LDL-C [2,3]. Therefore, these findings are of clinical relevance and support the clinical applicability of RC in this setting.

Tangier disease in Israel; Complicated by mobile lipomatous mass on mitral valve and acute stroke
Background and Aims: Tangier disease (TD) is a rare genetic disease which causes very low or even deficient HDL in plasma and accumulation of cholesterol in different tissues throughout the body. The disease is related to early onset of atherosclerotic disease and other clinical sequelae, previous articles described increased incidence of stroke in tangier patients. Valvular involvement in Tangier's disease was never described before .

Prediction of the 4-Year Incidence Risk of Ischemic Stroke in Healthy Japanese Adults: The Fukushima Health Database

Pharmacological interventions for asymptomatic carotid stenosis
CONCLUSIONS: Although there is no high-certainty evidence to support pharmacological intervention, this does not mean that pharmacological treatments are ineffective in preventing ischaemic cerebral events, morbidity, and mortality. High-quality RCTs are needed to better inform the best medical treatment that may reduce the burden of carotid stenosis.
