Finding balance (or I hate people now, need exposure to nonsucky ones)
I have grown to dislike people more and more over the last few years. Just tonight we got 2 meth abusers, 2 polysubstance, and an alcohol pancreatitis in the ICU. Every…single….admit/consult…. was drug related. Anyone have advice on balancing this **** out with volunteering or at least being exposed to the other side of society? The non suck side? Does it exist? I need more balance. (Source: Student Doctor Network)

Do you guys use NG or NJ for patients with severe pancreatitis not tolerating oral intake. Seems from uptodate you can use either. What are your thoughts? (Source: Student Doctor Network)

Drunk healthcare professional checks into the ER immediately after work. What would you do?
Had this case several months ago of a guy that got off from work as an OR nurse and came directly to the hospital, per him, for epigastric pain. He works at another hospital within our system. He has a history of pancreatitis due to “unknown cause”. He seems off and I suspect he is drunk. His alcohol level is 300 and lipase is markedly elevated. I inform him that he is drunk, and alcoholism is likely the cause of his recurrent pancreatitis. He of course says that’s impossible and must still... Question about pancreatitis
Wanted your opinions on your management. If you have someone come in with mild pancreatitis unknown etiology (full work up normal) with no abnormal labs other than the lipase being >5000. Vitals stable. APACHE II on admission 3. And patient wants to eat the next day with pain improved despite lipase being still >5000. Would you initiate early refeeding or would you wait a while? Alcoholism and the ER
Anyone think that the ER probably does more to allow people to continue drinking alcohol than to stop drinking? Took care of a guy today who has been to the ER numerous times, and admitted numerous times, for complications from EtOH. The very first thing he said was "I having pancreatitis from my drinking." He was honest, and I said I appreciated his honesty. Now, what would happen 100 years ago was these people would sit around at home in pain for several days, vomiting, and go... Possible administration discharge due to failure of ADAPT
So I will make this short, I am possibly facing an administration discharge for failure of ADAPT program for ETOH use. The back story is that last year I was diagnosed with Pancreatitis and the Air Force determined the cause was for excessive ETOH abuse. Forced me to enroll in the ADAPT program and sent me to rehab. I successfully completed rehab and was sober for almost 6 months until I had an relapse episode due to family situations. So now the ADAPT wants to fail me because of program... Does this patient have pancreatitis?
Have a patient who had N/V, diarrhea and epigastric pain. CT scan with contrast showed diffuse mild thickening of colon and small bowel representing enterocolitis; pancreas had no abnormality. Lipase 7000s and patient did have epigastric tenderness on exam. Is this consistent with pancreatitis or does the ct scan rule it out? (Source: Student Doctor Network)

Third spacing and fluids
Wondering how you all approach those complicated patients who are total body overloaded but intravascularly dry, as there doesn't seem to be a great deal of evidence based medicine to guide us. Let me give an example- patient with bad pancreatitis and hypoalbuminemic with an oxygen requirement and maybe a small effusion but otherwise not too terrible an Xray, who is persistently oliguric and has worsening renal function. Common practice seems to be to keep giving fluids with the aim of... Third spacing and fluids (Source: Student Doctor Network)

why high carb, low fat diet in chronic pancreatitis?
Anyone know why high carb, low fat diet is recommended in chronic pancreatitis? Surely both carbs and fat are bad in pancreatitis. Thanks in advance. (Source: Student Doctor Network)
