Sorry some dumb medical questions from a PGY1
I have some really dumb questions, if anyone would be willing to answer them!
1. Do you hold all GDMT or just the beta blocker during heart failure exacerbation? What about Farxiga? Is Amlodipine or Nifedipine contraindicated in HFrEF/HFpEF?
2. Are there any formal guidelines on when to start pharmacology for weight loss, such as Phentermine-topiramate?
3. How do you know if a chest tube is currently to suction, on water seal, or clamped?
4. How exactly do you diagnose DKA? Do you need...

Patient can't stop moving while under GA! SSRI implicated?
62 y/o F, on Cymbalta x 25 years and amlodipine x 5 years for htn. Nervous Nelly in OR. Knee scope, surgeon likes GA.
Easy everything. At 1.0 MAC Sevo for start of operation. Pt moves. I do the usual (prop bolus, increase sevo), surgeon
tries again, again patient moving like crazy. Again, I deepen-pt still moving! Fed up with looking like a spam sandwich,
I give 30 mg rocuronium, after which she finally stops moving. What gives? Only thing I could think of was the Cymbalta-I
vaguely...

doctor, why has my blood pressure gone bad?
I saw a lady in the clinic who asked me this question - she has diabetes and ischaemic heart disease .
Her blood pressure has been on average 145/ 85 -90 on perindoril 8mg, amlodipine 10 mg and a small dose of thiazide. Suddenly on home monitoring her BP readings are up to 210 / 120 . Her machine has been tested. she is concordant with her medicines and nothing else has happened in terms of symptoms / medicine changes. Any thoughts?

what do you think of doxazosin?
I saw a gentleman yesterday who has Ischaemic heart disease and hypertension and normally takes ramipril 10mg and amlodipine 10mg for hypertension. He was seen in the renal clinic last week as he now has stage 3 CKD and had a bp recorded at 220 / 90 (don't know if this was a one-off measurement or rechecked by the doctor) and was prescribed doxazosin 4mg. His BP yesterday was about 80 systolic and he has been feeling dizzy funnily enough. What do you think of this clinical hisotry?

A 76 years old man, type 2 diabetic for 10 years, on 1.0 gram of Metformin, HBA1C of 8%, BMI of 28.5, No Microalbuminuria, small right carotid intima thickness but no plaques. He has a healthy Life Style with regular exercises and Diet, taking also 10 mg Atorvastatin, and on Amlodipine 5 mg/day had a 24 hour Arterial Blood Pressure Monitoring requested by his GP because of his Hypertension. These are the Main Results Report:
- Mean 24 hours Systolic BP: 144 mmHg.
- Mean 24 hours Diastolic BP : 84 mmHg.
- Mean Systolic Daytime BP: 146 mmHg.
- Mean Night time Systolic BP 142 mmHg.
