The following excellent two article series published in 2013 give an in-depth view of the drug culture using Suboxone/buprenorphine, along with a history of the development of the drug. It is an eye opening account that shows the drug is approximately 1/6 as deadly as methadone, and that the death rate is not zero from buprenorphine. Methadone Rx
Hi everyone, There's a patient who has been filling methadone (~12 tabs/day= 120 mg) and klonopin at the pharmacy for almost 3 years. Upon review of the state's PDMP report, that's 369 mgEq/day. Per the dr's office the diagnosis code is M54.5 for chronic low back pain. I am relatively new to the pharmacy, and am not feeling comfortable filling the script. Under what circumstances, if any, would you fill the Rx? What if I don't fill it and the patient ends up going through withdrawal...? Any... Methadone Rx (Source: Student Doctor Network)

Would you fill these rxs?
Hi everyone, Would you question/fill these rxs? Scenario 1: Only medications on file are ambien cr 12.5 mg (#90), norco 10 mg (#180) and soma 350 mg (#120). They are all written by the same doctor. Scenario 2: Is prescribed soma 250mg and oxycodone 15 mg (#120) from the same doctor, who is a visiting physician (i.e sees patients at their homes.) Do you do much as an addiction psychiatrist?
I write this not to be disparaging, but just because I am a clueless intern with little experience. A little background: One of our rotations as interns (probably the best, in terms of hours certainly, but also cool attendings) is at the VA substance abuse program. It's basically an intensive outpatient program that meets within the hospital. Anyway, besides prescribing some naltrexone or methadone, I didn't really see us doing much. holding doctors accountable
I am very annoyed with other pain clinics in the area who are very lax on their prescription habits, e.g. very high MEDs (high doses of roxies, dilaudid, opana, fentanyl, methadone etc.) in a non-cancer pain setting and commonly without profound severe pathology of non-CA origin, and with other risky medications combinations (xanax, ambien, soma, etc.). Also, over and over I have noticed some of this patients to have very busy PMP (ER visits, different pharmacies, date overlaps),... holding doctors accountable (Source: Student Doctor Network)
