Is gabapentin and/or lyrica safe in pregnancy and/or breastfeeding patients?
Reviewing the literature online, seems to be somewhat limited in recommendations:

Burning Feet
I wanted to pick the collective mind of IPM on SDN.
I have an 85 year old female. Chief complaint burning pain of bilateral feet (specifically the soles). Pain is worse at night.
Worked up relatively well by her PCP, podiatrist, and two neurologists.
Has had adverse effects with gabapentin, Lyrica, and duloxetine.
BMP, CBC, B12, folate, TSH, A1C, all unremarkable.
EMG of upper and lower extremities shows sensory neuropathy and what is most likely mild carpal tunnel of the wrist...

Chronic pain
90 year old patient with chronic low back (prior lami/fusion) and continued knee pain after TKA. I’m the 3rd PM doc she’s seen. She failed to follow up with the other PM bc she says nothing was helping. per chart review she’s been tried on multiple meds(meds all with side effects or no relief: gaba, Lyrica, cymbalta, savella, Tylenol #3, Vicodin, oxycodone) I’m hesitant to try TCA at her age and she has afibb. 47 y.o. female book-keeper with right basilar thumb OA underwent non-image guided PRP injection 6 months ago. Felt excruciating and electrical sensation over dorsum of thumb during shot. Severe swelling at @24 hours in thumb and index finger. PCP did kenalog injection at 48 hours without improvement. Repeat kenalog injection at 2 weeks no improved. Who manages fibromyalgia?
I'm sorry if this has been discussed elsewhere, but in y'alls practice setting, who manages fibromyalgia? I'm starting to get more and more of these patients with diffuse muscle and joint pain, especially now that I just started seeing medicaid, yay. Typically I will refer to PT and a pain psych consult for cbt (which >50% never go to, and I'm in a small city with no good psych so refer outside), maybe start gabapentin/lyrica/cymbalta.

Preop opioid sparing meds
Do other people use preopedative meds significantly and feel it offers much? We routinely use Tylenol preop, plus or minus some Lyrica or gabapentin, I can’t say anecdotely I think it offers much though. Thoughts? (Source: Student Doctor Network)
