Improvement and International Validation of the Predictive Probability of the Patient Demographics, Radiographic Index, and Surgical Invasiveness for Mechanical Failure (PRISM) Model for Preventive Procedures in Adult Spinal Deformity Surgery
Study Design. This is an international multicenter retrospective review of 219 surgically treated consecutive adult spinal deformity (ASD) patients who had a minimum of five fused segments, completed a 2-year follow-up. Objective. The purpose of this study was to add the indices of preventive procedures to improve and to validate the predictive probability of the PRISM (patient demographics, radiographic index, and surgical invasiveness for mechanical failure) for mechanical failure (MF) following ASD surgery. Summary of Background Data. The PRISM was developed from the data of 321 ASD patients, which str...

Risk Factors for 30-day and 90-day Readmission After Lumbar Decompression
Study Design. Retrospective cohort study. Objective. To assess readmission rates and risk factors for 30-day and 90-day readmission after elective lumbar decompression at a single institution. Summary of Background Data. Hospital readmission is an undesirable aspect of interventional treatment. Studies evaluating readmissions after elective lumbar decompression typically analyze national databases, and therefore have several drawbacks inherent to their macroscopic nature that limit their clinical utility. Methods. Patients undergoing primary one- to four-level lumbar decompression surgery were retr...

Pain Relief After Selective Nerve Root Block as a Predictor of Postoperative Functional Outcome in Patients with Degenerative Lumbar Spinal Stenosis Patients Undergoing Decompressive Surgery
Conclusion. The degree of improvement in symptoms 6 hours after SNRB can predict the degree of improvement in LLRP at 12 months after surgery. However, symptomatic improvement after SNRB does not predict postoperative functional outcome or quality of life. Level of Evidence: 4 (Source: Spine)

Effectiveness and Safety of Inelastic Versus Elastic Lumbosacral Orthoses on Low Back Pain Prevention in Healthy Nurses: A Randomized Controlled Trial
Conclusions. Six-month wearing of LSO showed neither a significant difference in preventing LBP nor causing adverse effects to participants. Level of Evidence: 1 (Source: Spine)

Association Between Family History of Surgically Treated Low Back Pain and Adolescent Low Back Pain
Conclusion. Our findings suggest that adolescents who report a family history of LBP have higher odds of frequent LBP irrespectively of a family history of back surgery. Level of evidence: 4 (Source: Spine)

The Genetic Association Identified Between Intervertebral Disc Degeneration and Associated Risk Factors Based on a Systems Biology Approach
Conclusion. Genetic association between IDD and risk factors in the general population was revealed by association network. Important gene-related molecular pathways and chemical drugs closely related to IDD have been found. Further study can provide guidance for the treatment and prognosis of IDD. The Influence of Simulated Low Speed Vehicle Impacts and Posture on Passive Intervertebral Mechanics
Conclusion. Functional spinal units exposed to the highest severity impact (11 g) had significant neutral zone changes, with increases in joint laxity in flexion-extension and anterior-posterior shear and decreased stiffness, suggesting that soft tissue injury may have occurred. Despite observed main effects of impact severity, no influence of posture was observed. Level of Evidence: N/A (Source: Spine)

Pedicle Subtraction Osteotomy Construct Optimization: A Cadaveric Study of Various Multirod and Interbody Configurations
Study Design. Fourteen cadaveric specimens were separated into two groups: (1) L3 pedicle subtraction osteotomy (PSO) with transforaminal lumbar interbody fusion (TLIF) or (2) lateral lumbar interbody fusion (LLIF). A 2-rod configuration (2R) was compared with two supplemental rod configurations: 4-rod (4R) with accessory rods (ARs) using connectors or 4R with satellite rods (SRs) without connectors. Objective. Compare PSO constructs with different rod configurations and adjacent-level interbody support. Summary of Background Data. Supplemental rods and anterior column support enhance biomechanical perfor...

Predictive Factors on Initial in-brace Correction in Idiopathic Scoliosis: A Systematic Review
Conclusion. The results of this systematic review indicate that increased curve flexibility is strongly associated with increased initial in-brace correction. Level of Evidence: 1 (Source: Spine)

Automated Differentiation Between Osteoporotic Vertebral Fracture and Malignant Vertebral Fracture on MRI Using a Deep Convolutional Neural Network
Conclusion. We successfully differentiated OVFs and MVFs based on MRI with high accuracy using the CNN model, which was statistically equal or superior to that of the spine surgeons. Level of Evidence: 4 (Source: Spine)

Associations Between Neck and Shoulder Pain and Neuropathic Pain in a Middle-aged Community-living Population
Study Design. A cross-sectional study. Objective. The present study aimed to investigate the prevalence of NeP in subjects suffering from NSP to reveal the impact of NeP on the health-related QOL (HRQOL) in middle-aged and elderly people with NSP in a health checkup. Summary of Background Data. No previous studies have established the relationship between NSP and NeP as potential risk factors contributing to a decreased QOL in the general population. Methods. The present study involved 203 participants (men: 84, women: 119; mean age: 63.3 yr). For each subject, anthro-pometric measurements, physica...

Decisional Regret Among Older Adults Undergoing Corrective Surgery for Adult Spinal Deformity: A Single Institutional Study
Conclusion. While the majority of older adults were appropriately counseled and satisfied with their decision, one-in-five older adults regret their decision to undergo surgery. Preoperative depression was associated with medium/high decisional regret on multivariate analysis. Level of Evidence: 4 (Source: Spine)

Can We Stop Distally at LSTV-1 for Adolescent Idiopathic Scoliosis With Lenke 1A/2A Curves?
Study Design. A retrospective study. Objective. To determine in which case one level proximal to last substantially touching vertebra (LSTV-1) could be a valid lowest instrumented vertebra (LIV), in which case distal fusion should extend to last substantially touching vertebra (LSTV), and to identify risk factors for distal adding-on. Summary of Background Data. Posterior thoracic fusion to save more lumbar mobile segments has become the mainstay of operative treatment for adolescent idiopathic scoliosis (AIS) with Lenke 1A/2A curves. Although previous studies have recommended selecting the LSTV as LIV, g...

Effect of the Implementation of a Surgical Care Bundle in the Incidence of Surgical Site Infection in Spine Surgery: A Quasi-Experimental Study
Conclusion. After implementation of the care bundle, the incidence of SSI in spine fusion surgery decreased significantly. Multivariate analysis showed that the care bundle was an independent protective factor. The implementation of these measures should be reinforced on the routine medical practice to reduce the SSI incidence. Level of Evidence: 3 (Source: Spine)
