How to best handle vaccine decliners: scientific facts and psychological approach
There is currently no curative drug therapy for COVID-19. The spread of the virus seems relentless despite the unprecedented epidemiological measures. Prevention remains the only feasible option to stop the pandemic; without population-level vaccination, we are unlikely to regain the quality of social life and the unrestricted economy/commerce we enjoyed before. Anti-vaxxers and conspiracy theorists are seemingly oblivious to the detrimental effect of COVID-19 both at an individual and societal level. These groups have (and probably will) continue to attempt to undermine efforts to eradicate the virus despite the fact that...

Clinical and prognostic significance of elevated ferritin levels in hospitalised adults
Conclusions In hospitalised patients, ferritin levels higher than 2000 ng/mL are mainly associated with infectious and malignant diseases but do not predict mortality. (Source: Postgraduate Medical Journal)

In situ simulation training for a better interprofessional team performance in transferring critically ill patients with COVID-19: a prospective randomised control trial
Conclusion In situ simulation training using HFS significantly showed better performance than the standard training using LFS in regards to overall transfer and communication skills as well as teamwork performance. The training using HFS may provide a valuable adjunct to improve interprofessional skills, communication and teamwork performance in transferring critically ill patients with COVID-19. Trial registration number NCT05113823. (Source: Postgraduate Medical Journal)

Knowledge and practice of home blood pressure monitoring 6 months after the risk and assessment management programme: does health literacy matter?
Conclusion Patients could retain HBPM knowledge better after RAMP-group than RAMP-individual. Older, retired and patients with adequate health literacy were more likely to continue weekly HBPM 6 months after education. Trial registration number NCT02551393. (Source: Postgraduate Medical Journal)

Different pedagogies for acquisition of knowledge and skill: a systematic review and network meta-analysis
Conclusions Our study demonstrates all educational methods have positive effects on knowledge acquisition, but education methods involving high fidelity virtual patient simulation are better at improving skill performance than other methods. (Source: Postgraduate Medical Journal)

Postpartum haemorrhage and associated factors among mothers who gave birth in South Gondar Zone public health facilities, Ethiopia: a cross-sectional study
Conclusions The prevalence of postpartum haemorrhage is high. The age of the participants, disrespectful maternity care, labour induction and augmentation, the prolonged second stage of labour and no antenatal care visit were independent predictors of postpartum haemorrhage. Endoscopy findings in patients on dual antiplatelet therapy following percutaneous coronary intervention
Conclusions GIB bleeding occurred infrequently in the patients post-PCI on DAPT. Risk assessment scores (such as Glasgow-Blatchford and Rockall scores) are useful tools to assess the urgency of OGD and need for endoscopic therapy. (Source: Postgraduate Medical Journal)

Retrieving the deleterious mutations before extinction: genome-wide comparison of shared derived mutations in liver cancer and normal population
Conclusions Deleterious mutations are suppressed in functionally important genes as well as in normal populations. This work deepened our understanding on how natural selection act on deleterious mutations by analogising the cancer evolution to species evolution, which are essentially the same molecular process but at different time scales. Down-regulation of miR-219-5p increase the risk of cancer-related mortality in patients with prostate cancer
Conclusions miR-219-5p can be used as a biomarker to predict death risk in prostate cancer patients. The mortality risk prediction model constructed based on miR-219-5p has good consistency and validity in assessing patient prognosis. (Source: Postgraduate Medical Journal)

Acute localised exanthematous pustulosis (ALEP) over penis secondary to lamotrigine
Introduction A male in early adolescence born to non-consanguineous parents was brought to dermatology outpatient department services with multiple pustular lesions over penis since 2 days. There was fever associated with chills and rigours. The patient is a known case of Lennox-Gastaut syndrome (multiple seizure types, electroencephalogram changes, intellectual impairment) on sodium valproate since 10 years. The patient was started on tablet lamotrigine 50 mg once daily in addition to sodium valproate 4 days prior to the lesions for the new onset of seizure episode. There was no history of irritant application. On cutaneo...

'Two hearts that beat as one: does love cause physiological synchrony?
If you are looking for a card to celebrate an anniversary or St Valentine’s day, you may well come across a selection featuring the well-known sentimental lines: ‘Two souls with but a single thought, two hearts that beat as one.’ These days the saying is probably best known from a song by the rock band U21 but in greeting cards and on the internet, it is usually attributed (wrongly) to the English romantic poet John Keats. Its true source is a play by the Victorian actress Maria Ann Lovell, called ‘Ingomar the Barbarian’,2 and adapted from the Austrian playwright Friedrich Halm. Both authors d...

BLANKETS: a toasty tool to improve social history documentation for our older patients
A thorough social history is an important component of all medical clerkings and is particularly crucial when admitting an older patient.
